| Objective Inquire into a new type of rigid pipe line feasibility analysis in aortic root replacement, testing the feasibility of the method whether it can saving time, reduce anastomotic bleeding and affect the supply of coronary blood flow.Methods Select10dogs who are in good health, these dogs whose weight are amount20kg, and ages are2-5years old. Experimental group:5dogs who are used a new type of rigid pipe line fit aortic, control group:5dogs who are used normal aortic anastomosis. All the animals-chest were opened after tracheal intubation general anesthesia, and then perfusion cold crystal liquid outer loop in order to establish cardiopulmonary bypass. The test group:adopting the method of free suture anastomosis. The new type of rigid pipe line is mainly composed with a recent heart side of ring and a distant heart side of ring, artificial blood vessel, left hole, right hole. Both hard ring has a width of groove. The basic method:after the experimental dog whose cardiac arrest, the aorta were cut off at lcm above sinotubular junction. Looking for intelligent aorta ring, measuring the distance between left and right coronary artery opening, to punch holes in artificial blood vessel with the corresponding position of coronary artery opening, and then set a rigid ring into artificial blood vessel at lcm above coronary artery opening. Using artificial blood vessel which near heat to suture aortic ring. Ligaturing and fixing recent heart side of ring with No.7black silk.Shearing corresponding length of the artificial blood vessel, after setting a rigid ring which distant heart side into artificial blood vessel, Ligaturing and fixing ring with No.7black silk. Blood flow to rigid pipe and vessel wall in the lacunar, via coronary artery open separately into the left and right coronary artery reperfusion myocardium.The control group:after the experimental dogs’cardiac arrest, the ascending aorta were Cut off, using artificial vascular to anastomosis aorta, coronary artery fenestration on the corresponding, use the anastomosis of coronary arteries open.Record the time, the content of cTnl, the heart rate and the mean artery pressure after cardiac complex jump, Observe whether the Anastomotic happen to bleed.Results Test group:The preoperative test group of mean cTnI content:1.13±0.02ug/L. the test group5hours after surgery of cTnI content:1.13±0.02ug/L. animals were putted to death and the ligation of aortic tissue were sentted pathological biopsy5hours after, the results suggest Aortic wall did not see obvious ischemia, necrosis., measuring the CPB time:14.20±0.83min,; the aortic clamping time:9.00±1.00min; and the aortic ligation anastomosis of test group:7.80±0.83min. observing the mean heart rate:173.00±6.67/min; and the mean artery pressure107.8±2.86mmHg. The control group:measuring the CPB time:39.40±5.17min; the aortic cross-clamping time:29.80±3.63min; and the aortic anastomosis time:28.60±3.78min. observing the mean heart rate:171.40±5.50/min; the mean artery pressure:106.8±3.42mmHg. The preoperative control group of cTnI content:0.12±0.02ug/L, the control group5hours after surgery of cTnI content:1.10±0.04ug/L. After open cycle of canine all the hearts were successful resuscitation. The anastomotic bleeding of test group did not see obvious, there is one group of control group that coronary artery tone timeliness are difficult to stop the bleeding.Conclusion The new type of rigid pipe line can implement aortic root replacement and the method can not only obvious saving time, but also reduce anastomotic bleeding, the method didn’t affect the supply of coronary blood flow. |
