| Objective: To compare and analyse the diagnostic value of magnetic resonanceimaging (MRI) and arthroscopy for the meniscus injuries to further clarify the operativetestify of the arthroscopy for the meniscus injury. Methods:116cases hospitalizedpatients in our hospital were collected from February in2011to March in2013. They wereClinically diagnosed knee joint meniscus injury, and simultaneously done the MRI andarthroscopy. The arthroscopy was taken as the gold standard and the SPSS17.0softwarewas used for statistical analysis to Calculate the sensitivity, the specificity, the positivepredictive value, the negative predictive value, Youden index, the missed diagnosis andmisdiagnosis rate of MRI in the diagnosis of meniscus tear. Kappa value was adopted totest the effectiveness of MRI in the diagnosis of meniscus tear.The chi-square analysis wasused to evaluate the MRI and arthroscopy check whether there was any difference on themeniscus tear, and to compare differences of MRI in the diagnosis of the lateral meniscusof the knee joint tear. According to the meniscus MRI signal, the patients were dividedinto three groups. The first division MRI performed â… l evel signal in the meniscus.Thegroup B showed â…¡ level signal, and group C showed â…¢ level signal.The chi-square wasneeded to analyse differences among three groups whether to do the arthroscopic surgery.Results: The sensitivity (Sen) of MRImeniscus tear was92.4%. The specific degrees (Spe)was96.0%, the positive predictive value (PV (+)95.1%, the negative predictive value (PV)93.7%, the misdiagnosis rate4.0%, the missed diagnosis rate7.6%, consistent rate94.3%,and Youden index was89.4%. The effectiveness of the Kappa value of MRI for meniscustear diagnosis was0.887. The sensitivity of MRI for the medial meniscus tear was94.2%and the specificity was93.7%. The sensitivity of MRI for the lateral meniscus tearwas90.7%and the specificity was98.3%. It had no statistical difference to analyze thesensitivity (X~2=0.098, P>0.098), specificity (X~2=0.38, P>0.38), consistent rate (X~2=0.081, P>0.081) of MRI diagnostic about the medial and the lateral meniscus tear Bychi-square test.It had significant differences between group C and group A (X~2=117,P<0.005), group C and group B (X~2=112, P<0.005) needed the operation treatment.Butthere were not any significant differences between party A and party B (X~2=0.041,P>0.041). The operation rate of MRI â…¢ signal for the meniscuswas97.1%. Conclusion:MRI is an effective method to diagnose the meniscus tear. It should be routine to do MRIexamination for considering the meniscus injury patients. There are not any significantdifferences between the examination results of MRI and the arthroscopy. It should betreated conservatively for â… and â…¡ level signal.but â…¢level signal of MRI performanceshould do arthroscopy as early as possible. |
