Background:Pancreatic neuroendocrine neoplasms is a very rare kind of pancreatic tumor.In this research, we investigated the clinical characteristics of this disease based on44cases, and discussed the protocols of surgical diagnosis and treatments.Methods:From Dec.1994to Dec.2012,44patients with the diagnosis of pancreatic neuroendocrine neoplasms were treated in Sir Run Run Shaw hospital, affliated with Zhejiang University. They all received surgical operation, and had pathological diagnosis as pancreatic neuroendocrine neoplasms. Their clinical data were analyzed in this study.Results:The patients consisted of17males and27females, with their age ranging from22to71years. The time for symptoms varied from7days to12years. All patients received surgical treatments, which included partial resection of the tumor in12cases, distal pancreatectomy in23cases, middle pancreatectomy in2cases, pancreaticoduodenectomy in7cases. Five patients had liver metastasis. Among them, three cases have metastatic tumor resected, two cases had trans-catheter arterial chemoembolization, since their multiple metastasis were unresectable. The diameter of tumor ranged from0.8to11.0cm, with the average of4.5cm. Pathological grading showed that there were31cases of P-NEN G1,6cases of P-NEN G2(6cases with liver metastasis,2cases with lymph node metastasis),7cases of P-NEC (4cases with liver metastases,3cases with lymph node metastasis,1cases with invasion with duodenum). During follow-up,39cases still survive, and five cases were dead. All37cases of P-NENS (GI, G2) survived.In7cases of P-NEC cases,2patients survived, and five cases were dead.Conclusions:Pancreatic neuroendocrine neoplasms has a better prognosis than other kinds of pancreatic malignancies. Surgical resection is the basic of combined treatments. Biological therapy, molecule targeted therapy, radiofrequency ablation, chemotherapy were proved to be useful when they assisted surgery. |
