Objective With studying the effect of early enteral nutrition(EEN) for severe acutepancreatitis(SAP) patiens on hospitalization days、cost、serum C-reactive protein(CRP)、T lymphocyte subsets (CD3, CD4+, CD4+/CD8+ratio)、prealbumin(PA) andalbumin(ALB),then to evaluate the clinical significance of EEN in patients with SAP.Methods In our department, forty-nine SAP patients were randomly divided into theexperimental group(25cases) and the control group(24cases). The all patients wereinserted sacciform jejunum catheter after admission. The experimental group weretreated with early enteral nutrition(in48h after admission). The control group weretreated with early parenteral nutrition(PN), and EN was given after gastrointestinalfunction restored(Usually after admission7to9days). On the days of1、5、9and14after admission, the patients of two groups were testing serum CRP、T lymphocytesubsets (CD3, CD4+, CD4+/CD8+ratio)、Prealbumin(PA) and albumin(ALB). Thehospital stay and the total cost were reckoned. All data were analyzed by usingchi-square test, analysis of variance(ANOVA) for repeated measurement andindependent samples t-test.Results Four pantiens were excluded experiment, and the others42cases were cured.The average hospitalization days and cost were lower in experimental group than that incontrol group(P<0.05). The serum CRP and T lymphocyte subsets levels, on the firstday, were not statistically significant differences in two groups(P>0.05). On the5th,9th and14th hospitalization days, the serum CRP level was lower in experimental group than that in control group, and with treatment, the serum CRP levels in two groups weredecreasing change (P <0.05); the T lymphocyte subsets (CD3, CD4+, CD4+/CD8+ratio) levels were higher in experimental group than that in control group, and withtreatment, T lymphocyte subsets (CD3, CD4+, CD4+/CD8+value) levels wereincreasing change(P <0.05). PA and ALB, on the first hospitalization day, were notstatistically significant differences in two groups(P>0.05). As the treatment, on the5th、9th and14th hospitalization days, the serum PA and ALB were higher inexperimental group than that in control group(P <0.05).Conclusions1、This experiment shows that EEN by sacciform jejunum catheter forSAP patients could reduce hospitalization days and cost.2、EEN was better in reducinginflammation on the acute reaction period, and was better in improving immunefunction and nutritive condition.3、The treatment of EEN for SAP patients has apositive clinical significance.
