BANCKGROUND: For of adjacent segment disc degeneration after lumbar fusionresearch a lot, but opinions vary, some study results contradict each other, in order tofurther understand the related factors of degeneration, from the Angle of the imagingfound that the relationship between various factors, this article followed up a lot ofpatients after lumbar fusion, retrospective analysis of segmental lumbar fixation andimaging correlation of adjacent segment disc degeneration and to guide clinical work.OBJECTIVE: Postoperative lumbar fusions were followed for the MRI manifestationof adjacent segment disc degeneration, discuss the correlation between them.DESINGN,TIME AND SETTING: Retrospective case analysis, in a March2012inSeptember2010in the first affiliated hospital of anhui medical university.PARTICIPANTS: Choice in the first affiliated hospital of anhui medical universityparallel MRI of268patients with lumbar, male152cases,116cases of women; Aged40to65years old, average52.8years. According to all quasi fixed fusion in patientswith preoperative adjacent intervertebral disc whether degeneration will all patientswere divided into degeneration group (group A) and the degenerationgroup (group B).METHODS: In March2006a268cases in our hospital in March2010after lumbarspinal internal fixation fusion treatment lumbar segmental fixation fusion adjacent disc degeneration and postoperative MRI performance, to explore the correlation betweenthem.MAIN OUTCOME MEASURES: According to Pfirrmann grading will degree ofintervertebral disc degeneration from light to heavy is divided into five levels, accordingto JOA score evaluation.RESULT:1. The communist party of China (144cases (53.7%), postoperative vertebralfusion adjacent segment degeneration,39cases were single segmental fusion,262cases of segmental fusion,3segmental fusion of43cases.144for adjacent segmentdisc degeneration MRI score:369cases,24cases of four points, five points of51cases.As shown in table1, two groups of patients age, sex ratio, follow-up time, number offusion segments, JOA score at each time point and the adjacent intervertebral discexpression had no statistical difference (P>0.05). Patients all appear degenerationgroup A, group B70cases appeared back into. MRI performance comparison of twogroups, group A degeneration degreeis big, fast speed, while group B had degeneration, but the degeneration is slow,we found that eight group B patients appeared degeneration,2years later finallyfollow-up degeneration extent reduce instead.2. Group A after degeneration of summary of the related factors ofpostoperative JOA score at2years no statistical difference (P>0.05),postoperative MRI manifestation of adjacent segment disc degenerationin the2years along with the increase in fixed fusion segments, MRI score levelsincreased gradually; Last follow-up JOA score reduced gradually with the increase offixed segment, MRI rating is higher, the difference was statistically significant (P <0.05), but the JOA scores and MRI manifestation of intervertebral disc degeneration is not consistent.CONGCLUSION: Lumbar degenerative disease fixed adjacent segment discdegeneration appears after the treatment, is associated with age, preoperative existing oflumbar disc degeneration and adjacent segment degeneration rate of the preoperativeand fixed segment about the degeneration accelerated, but the intervertebral discdegeneration of no obvious relationship between MRI manifestations and clinicalsymptoms.
