Research On Curative Effect Of Yougui Pill On Patients With Steroid Resistant Nephrotic Syndrome And The Influences On GRα And GRβ

Objective: To investigate the Yougui Pill on steroid resistant nephrotic syndrome (SRNS)clinical curative effect and its mechanism of action.Methods: The first stage: Patients with nephrotic syndrome (PNS) were randomlyincluded into the study, all of whom were in accordance with the test condition. The patientswere divided into3groups according to the treatment effect of1-month standardglucocorticoid (GC) treatment. The SSNS group consisted of13patients withsteroid-sensitive nephrotic syndrome (SSNS); the SRNS group included17SRNS patientswith Yang deficiency based on syndrome differentiation, and10health people were selectedas the health group. The three groups were determined for the expressions of GRα and GRβ inperipheral venous blood mononuclear cell (PBMC), which were compared and analyzed. Thesecond stage: Then the17SRNS patients were randomly assigned into two subgroups: onewas the control group (n=8) and the other one was the treatment group (n=9). The controlgroup was received conventional GC treatment for1month, while the treatment group wasgiven Yougui Pill for1month in addition to the treatment as in the control group. Theirclinical symptoms and signs were recorded and rated. And24hours urinary protein wasdetected. Finally the therapeutic effects of the patients in the two groups, as well as theexpression changes of GRα and GRβ were compared and analyzed.Results: Yougui Pill Combined with GC in the treatment of SRNS patients in clinicalsymptoms,24hour urinary protein quantitative improvement effect is better than the purewestern medicine treatment (p <0.05). GRα expression was higher in the health group andSSNS group than that of the SRNS group (p<0.05), while GRβ expression was lower in thecontrol group and SSNS group than that of the SRNS group (p<0.05); GC can down-regulatethe GRα (p<0.05), while Yougui Pill can up-regulate the GRα in SRNS group (p<0.05); BothGC and Yougui Pill showed no significant influences on GRβ expression (p>0.05);Combination of Yougui Pill with GC treatment had better curative effect on SRNS thansimple western medicine treatment.Conclusion: Yougui Pill to improve clinical symptoms of patients with SRNS (Yangdeficiency) and24hour urine protein content decreased significantly, and its mechanism maybe related to the up regulation of GR α expression.
