Compare The Changes Of Cellular Immune Function And IL-6in Patients Undergoing Laparoscopic Surgery For Colorectal Cancer By Two Different Of Anesthesia Protocols In The Perioperation

Objective:To investigate the effects of conventional general anesthesia or Epiduralanesthesia compound protocols on perioperative contents of T-lymphocyte subsets、NK cell and IL-6in patients undergoing laparoscopic surgery for colorectal cancer.Discussing anesthesia methods contribute less to cellular immunity. To provide thebetter choice of anesthetic method for laparoscopic colorectal cancer surgery.Method:Thirty patients undergoing laparoscopic surgery for colorectal cancer in ourhospital in the2012were randomly divided into two groups. The conventionalgeneral anesthesia in group A and the Epidural anesthesia compound in group B.Keep the epidural anesthesia in group B in the operation. Observation records of thepatient of surgery and anesthesia medications. Postoperative analgesia were samemethods, and VAS scores for the patient in PACU. Venous blood samples were takenat the before anesthesia(T1),2h after skin incision(T2),at the end of operation(T3),1dafter operation(T4) and3d after operation(T5). T-lymphocyte value(CD3~+、CD4~+、CD8~+、CD4~+/CD8~+) and natural killer cell(CD3—/CD16+CD56+) were determined byflow cytometry instrument. Interleukin-6was determined by enzyme-linkedimmunoassay. Compared two groups of immune indexes.Result:(1) T-lymphocyte subsets in two groups. The levels of CD3~+、CD4~+、CD4~+/CD8~+were all decreased from T2in both of groups. And these indicators were significantlydecreased at T3-T5compared with T1in group A（P<0.05）;The level of CD3~+wassignificantly decreased at T3and CD4~+、CD4~+/CD8~+were significantly decreased atT2-T5compared with T1in group B（P<0.05）. The levels of CD3~+、CD4~+、CD4~+/CD8~+in group B is higher than the group A. The levels of CD3~+、CD4~+weresignificantly different at T4compared with two groups（P<0.05）;and the level ofCD4~+/CD8~+was significantly different in T4in two groups（P<0.05）. There were nosignificant differences in the levels of CD8~+at any times between two groups or compared with T1.(2)Natural killer cell in two groups. There were no significant differences in thelevels NK cells at any times between two groups or compared with T1.(3)Interleukin-6in two groups. The level of IL-6was significantly decreased atT2-T5in two groups. And group B is significantly higher than group A at T2-T4.Conclusion:There were inhabition of perioperative cellular immune function in patientsundergoing laparoscopic surgery for colorectal cancer in both of groups. However,Epidural anesthesia compound has less inhibitory effect on perioperative cellularimmune function and IL-6than conventional general anesthesia in patientsundergoing laparoscopic surgery for colorectal cancer. So to patients moreadvantageous.
