Compare the effect of pedical fascial flap and biomembrane as the membrane guided material which wrap the unceller tissue engineering bone respectively, using membrane guided bone regeneration techniques(MGBR) to repair the exceed critical size bone defect of New Zealand white rabbits ulna, and observe and quantify the repairing effect.Constructed a one centimeter long bone defect model with periosteum in the middle of both ulnas of30New Zealand rabbits(male or female). The uncellular tissue engineered bone which was implanted into the bilateral bone defect areas was prepared by seeding the autologous red bone marrow(ARBM) on the osteoinductive absorbing material containing bone morphogenetic protein(BMP). The left side of bone defect area was wrapped by biomembrane as control group. The right limbs were wrapped by pedical fascial flap, which was a satisfactory membrane guided material with abundant capillary network and the effect of membrane guided bone regeneration. The right side was wrapped by pedical fascial flap as experimental group. The two groups were performed with internal fixation of mini titanium plate. In the fourth,eighth, twelfth, sixteenth week after the operation, each group was examined by X-ray radiograph, then executed6animals respectively to draw materials to conduct gross morphology, histological examination, and bone shape measurement analysis in the reparative area.3animals were executed on the12th and16th weeks to perform the biomechanical detection.by the observasion of X-ray radiograph,the recanalization of cavum ossis,the degradation and absorption of the implant, the quantity and the forming speed of external callus in the gross morphological inspection, bone trabecula, cartilaginous tissues and mature bone structure in the histological examination,the remodeling of diaphysis structure. On the16th week, X-ray radiograph of experimential group showed that bone density of the area of bone defect was high, normal diaphysis construction was formed and had a nice moulding, and cavum ossis was recanalization completely; the gross morphological inspection showed that callus on the surface of the implant had a better moulding, which was similar to os integumentale, and fascial flap formed amber periosteum tissue, whose quality was tenacious; the histological examination showed that the implant had been degradated, absorbed and substituted completely by new bone creeper, in which a small quantity of cartilage developed, and muture bone architecture formed and cavum ossis was recanalization completely. All of them were superior to control group. By the analysis of the bone shape measurement, the differences at the ratio of new bone trabecula area to total reparative area among three groups and the differences at the different time points at the fourth,eighth, twelfth, sixteenth week after the operation were statistically significant(P<0.05). By the comparision of the biomechanical measurement, the differences between the two groups at the twelfth and sixteenth week after the operation and the differences of the same group at each time point had statistical significance(P<0.05).By the comparision of the effect of biomembrane and pedical fascial flap in membrane guided bone regeneration effect, the latter has obvious curative effect in repairing the exceed critical size bone defect associating with the technology of uncellular tissue engineered bone.
