Objective:To compare the early clinical results of the treatment of degenerative lumbarspinal stenosis with the dynamic stabilization system in addition to decompressionand fusion, fixation in addition to decompression.Methods:72cases are statistical analyzed for the patients with degenerative lumbarspinal stenosis income of the Affiliated Hospital of Shandong University ofTraditional Chinese Medicine spinal orthopedics operation therapy from October2010to October2012,patients according to the selected operation is divided intodifferent: dynamic stabilization system in addition to decompression (group A) andfusion, fixation in addition to decompression (group B).The group A consisted oftwenty-two patients, and the group B consisted of fifty patients. All the patientswere followed up for a mean of13months(range,6~24months).The factorsconsidered included the operative time, amount of intraoperative bleeding, pre andpost operative assessment based on pain visual analog scales(VAS)、postoperative JOA score and the Oswestry disability index(ODI).ResultsThe final valuation was based on the documents of the last fellow up. The twogroups had significant differences on operative time, amount of intraoperativebleeding, the group B is higher than the group A(P<0.05).There were no significantdifferences in the pain visual analog scales(VAS) and the Oswestry disability index(ODI), postoperative JOA score (P>0.05). In the two group, VAS and theOswestry disability index(ODI), improved from preoperative topostoperative(P<0.05).ConclusionCompared to the segment fusion and fixation, trans-pedicular dynamicstabilization for the treatment of degenerative lumbar spinal stenosis, have themerit of minimally invasive, shorter surgical time, little hemorrhage. But their earlyclinical effect were comparable, maintain the stabilization of the spine, prevent thedegeneration of the instrumented segments and adjacent segments. The two kindsof operation methods for the treatment of lumbar spinal stenosis, the recentfollow-up clinical efficacy without significant difference. Further long-term andrandomized studies are necessary to confirm our data....
