Objective:1. Tissue Doppler imaging was used to analyze characteristics of left ventriculardiastolic function and left ventricular wall motion synchronization of the normalgroup and patients with diastolic heart failure;2. To analyze the correlation of the left ventricular diastolic function anddiastolic synchrony.Methods:1.Conventional two-dimensional ultrasound and Tissue Doppler Imaging wereused to measure the left ventricular wall motion velocity, mitral early diastolic flowvelocity (E), late diastolic flow velocity (A),early diastolic myocardial velocity (Em),late diastolic myocardial velocity (Am) from the apical four chamber view, andcalculate the E/Em on behalf of diastolic function of55patients with DHF and55normal people;2. M type echocardiography was used to measure the left ventricular ejectionfraction(LVEF) in left ventricular long axis view;3.Tissue Doppler imaging was used to measure the diastolic velocity andcorresponding time interval of left ventricular wall basal segment, middle segment,the apical segment in apical four chamber view, two chamber and on the long axisview, and calculate the left ventricular18segments in the standard deviation of earlymyocardial diastolic peak time (Te-SD), on behalf of the whole synchronization ofventricular wall motion; The delay of the early diastolic peak time in differentmyocardial segments--Anterior septum-posterior wall delay,Anterior wall-inferiorwall delay,Postal septum-lateral wall delay--represents the left ventricular regionaldiastolic dyssynchrony index.Results:1.Compared with the normal group,the diastolic function of patients withdiastolic heart failure decreased Obviously(P<0.01);2. The speed of basal segment,middle segment and apex of heart in the same left ventricular wall of normal group dreased by degrees;There was no significantdifference in dyssynchrony index,Te,of different fractions in the same leftventricular wall and different left ventricular walls in the same fraction(P>0.05);3. Compared with the normal group, dyssynchrony index of patients withdiastolic heart failure were significantly different (P<0.01);4. There was significant correlation between the index,E/Em, to evalate leftventricular diastolic function and dyssynchrony index,Te-SD, of patients withdiastolic heart failure(R=0.466,P<0.01);5. There was negative correlation between LVEF and Te-SD of patients withDHF (R=-0.535,P<0.01).Conclusions:The patients with diastolic heart failure often coexist with the left ventriculardiastolic asynchrony,and closely related with decreased diastolic function. TissueDoppler imaging can be used to evaluate the left ventricular diastolic function anddiastolic synchrony of patients with diastolic heart failure, and also can be used tobetter evaluate the correlation of left ventricular diastolic function and diastolicsynchrony. |
