PartⅠINTRATUMORAL SUSCEPTIBILITY SIGNAL OF BRAINGLIOMA: A CONTROL QUANTITATIVE STUDY BY ESWANObjectiveTo compare the relative signal intensity (RSI) of intratumoral susceptibilitysignal(ITSS), the size of ITSS in enhanced gradient echo T2star weighted angiography(ESWAN) between high and low grade cerebral gliomas. To evaluate the value of differentseries of MR to detect ITSS.To inivestigate characteristics of ESWAN in showing brainglioma.Materials and MethodsA series of MRI and ESWAN imagings of30patients with a pathlogic diagnosis ofglioma were included in the study.Magnitude imagings were reconstructed with functoolimage-disposal software at ADW4.3workstation.We got the minimum ITSS intensity,thesignal intensity of splenium of the corpus callosum, the size of ITSS in magnitudeimagings.RSI was calculated. The size of ITSS in DWI、T2WI and T2-FLAIR weremeasured also. Independent sample T test and Paired sample T test were used to analysethe differences of Quantitative Parameters between high-and low-grade glioma by analysissoftware SPSS17.0.At last,to reformat the magnitude imagings with the thickness of6millimetre and to observe the vasculars of tumors.ResultsRSI and the size of ITSS had significant differences between high and low gradecerebral gliomas. ESWAN、DWI、T2WI and T2-FLAIR have different sensitivity indetection of ITSS and their sensitivity fall in turn。ESWAN can show both arteriole and veinlet of certain tumors.Part ⅡCOMPARATIVE STUDY OF INTRATUMORALSUSCEPTIBILITY SIGNAL OF ESWAN WITH ANGIOGENESISIN BRAIN GLIOMAObjectiveTo analyse the correlation between intratumoral susceptibility signal by3D-enhancedsusceptibility-weighted angiography (ESWAN) and the morphology,microvessel densityand vasculogenic mimicry of vessels in brain glioma. To identify VM in brain gliomas andto explore its characteristics of ITSS. To investigate weither the imaging fingdings ofESWAN can express the angiogenesis seen under an electron microscope,looking forwardto providing more correct imaging information for diagnosis and treatment of brain glioma.Materials and MethodsA series of ESWAN imagings of thirteen glioma patients who underwent surgery inShan Dong province hospital were included in the study. Thirteen pathological sectionsobtained from the above patients were observed under an electron microscope after dualstaining for endothelial marker CD34and periodic acid–Schiff (PAS). Vasculogenicmimicry (VM) and microvascular density(MVD) were detected in thirteen cases ofgliomas. MVD as a indicator of glioma angiogenisis was acounted with corrected Weidnermethods.Vasculogenic mimicry in the glioma samples was determined by dual staining forendothelial marker CD34positive and periodic acid–Schiff (PAS) negative. And weanalysed the forms,the distribution of angiogenesis of different grade gliomas. Relativesignal intensity of introtumoral sensitive signal was calculated in magnitude imaging ofESWAN obtained with the Functool post processing software in ADW4.3-06workstation.The grade of glioma was determined according to the2007WHOclassification of tumours of the central nervous system. The association between MVD andRSI,MVD and the grade of glioma,VM and the grade of glioma,the forms of angiogenesisand grade of glioma were analyzed. Spearman rank relational coefficient was employedwith the statistical analysis software SPSS17.0to analyze the correlation above.ResultsThere was no association between MVD and the grade of glioma or RSI.Vasculogenicmimicry was present only in higher grade gliomas,while gliomas with VM often have higher RSI.In higher grade gliomas,the forms of vascular are many and varied,and theexpression of CD4and PAS varied greatly while ITSS of higher grade gliomas was oftensiganificant.ConclusionESWAN Imaging Quantitative Parameters, RSI and the size of ITSS are valuable inpreoperative grading of gliomas. The sensitivity of ITSS of ESWAN、DWI、T2WI andT2-FLAIR ITSS fall in turn.ESWAN can show edema around the tumor and some tumoralvascular.RSI of ESWAN can not represent the exsitence of MVD of gliomas,while higherRSI suggest that glioma got a higher grade and the exstence of VM as VM only exsit inhigh-grade gliomas.
