Objactive:To observe the effect of PHACO+IOL in the treatment of primary angle-closure glaucoma with cataract,providing the reference for the clinical treatment.Medthod:1.We selected25eyes,20cases of primary angle-closure glaucoma with cataract which underwent PHACO+IOL,5of them were pre-clinical stage,1Oof them were onset stage,10of them were chronic.They all have varying degrees of opacity of lens cortex and nucleus density increased.Then we compared of preoperative and3months postoperative, visual acuity, intraocular pressure, angle, depth of anterior chamber. The follow-up time was3to9months,1day,1week, lmonth,3month,6month after operation and the final follow-up.Result:1.120eyes(80%) acquired better eyesight (P<0.05), it is statistically significant differences.1.2Intraocular pressure(IOP) of5eyes of pre-clinical stage falled.(t=3.639, P<0.05)IOP of10eyes of onset stage falled.(t=8.720, P<0.01)IOP of10eyes of angle-closure glaucoma chronic falled.(t=3.386, P<0.05).All of them were statistically significant differences.1.325eyes with angle closure quadrant had varying degrees of openness (P<0.05), it is statistically significant differences.1.4Central anterior chamber depth of25eyes was increased after operation; peripheral anterior chamber depth of25eyes was increased after operation.(P<0.05), it is statistically significant differences.2. We selected21eyes,18cases of primary angle-closure glaucoma with cataract which underwent PHACO+IOL associate with glaucoma filtration operation,11of them were onset stage,10of them were chronic.They all have varying degrees of opacity of lens cortex and nucleus density increased.Then we compared of preoperative and3months postoperative, visual acuity, intraocular pressure, angle, depth of anterior chamber. The follow-up time3to9months,1day,1week,1month,3month,6month after operation and the final follow-up.Result:2.118eyes(86%) acquired better eyesight (P<0.05), it is statistically significant differences.2.2Intraocular pressure(IOP) of10eyes of onset stage falled.(t=3.495, P<0.05)IOP of10eyes of angle-closure glaucoma chronic falled.(t=3.667, P<0.05).Both of them were statistically significant differences.2.3Central anterior chamber depth of25eyes was increased after operation; peripheral anterior chamber depth of25eyes was increased after operation.(P<0.05), it is statistically significant differences.2.4We did not observe the angle closure quadrant.3. We did a statistical analysis on the differences of the intraocular pressure and visual before and after surgery between experimental group and the control group.Result.The vision independent samples t test:t=1.384, P=0.174>0.05, no statistical differences.Intraocular pressure independent sample t test:t=1.186, P=0.242>0.05, no statistical differences.Conclusion:PHACO+IOL in treating acute angle-closure glaucoma with cataract is effective, for most chronic angle closure glaucoma with cataract is effective.
