Yantai City Hospital Of Traditional Chinese Medicine Diagnosis And Treatment Of Acute Ischemic Stroke Delay Related Factors Analysis

Objective：This study intend to explore the delayed situation of diagnosis and treatmenton the patient with acute ischemic stroke in ShanDong Province YanTai city’s first classhospital of traditional Chinese medicine and to analyze the relevant factors leading todelays. Provide an objective basis for the development of practical interventions.Method：We collected a total of173patients with acute ischemic stroke from January toNovember in2012.These cases came from the ShanDong Province YanTai city’shospital of traditional Chinese medicine, Department of encephalopathy and theemergency ward. The object included demographic sociological information, the onsetsituation, treatment process and the treatment process of staying in this hospital etc.Patients of external of hospital were divided into≤5hours group and＞5hours groupaccording to the time from onset situation to arrived to hospital. Statistical analysisincluded used x2/t/wilcoxon rank test to analyze the collected data, P＜0.05(statisticallysignificant). Patients of internal of hospital were divided into≤1hours group and＞1hours group according to start treatment time. Statistical analysis included usedx2/t/wilcoxon rank test to analyze the collected data, P＜0.05(statistically significant).Result:1、the age of patients from53～87,the average age is （70.9±6.9）,the quantityof male patients are102（59%）,the quantity of female patients are71（41%）.The factorincluding Sex, place of residence, career, degree of education,living areas before onsethalf a year ago etc. Logistic regression showed the following factors were meaningful tostatistics. 2、the factor including patients’the first symptom,the means after the first symptom, thetime of decide to go to hospital after the first symptom,first hospital arrived, reason ofchoosing first hospital,the way to hospital,first department of this hospital. Logisticregression showed the following factors except reason of choosing first hospital weremeaningful to statistics.3、one of the first symptoms is disturbance of consciousness. Logistic regressionshowed the following factor was meaningful to statistics.4、ischemic stroke confirmed time，time of blood test，time of head CT test，time ofnervous system test and the NIHSS score of admission, time of get the CT result andblood test result. The following factors were the points of internal hospital treatmentdelay.Conclusion:The normalization treatment of early hyperacute ischemic stroke has already reached aconsensus among modern medicine all over the world. According to the standard oftreatment to ischemic stroke in western medicine. The phenomena of admission delay inhyperacute ischemic stroke is especially serious in ShanDong Province YanTai city’sfirst class hospital of traditional Chinese medicine. The idea of thrombolytic therapy isnot established. The public lack of awareness of first-aid when onset stroke symptom.Treatment procedures are not standardized and all the related departments does notcooperate closely. When face to early hyperacute ischemic stroke(<6h even <3h), thisconception still has more flaws. Thus to establish green channel, strictly controlthrombolysis indications and spread the knowledge of ischemic stroke is necessary forimproving the thrombolysis rate. Only then like this can make patients get the treatmentand benefit in time.
