| Objective:To summarize the clinical experience of surgical treatment for heart valvedisease with Native GK bileaflet mechanical heart valve.Methods: From January2006to March2010,52patients with heart valve disease wereperformed on heart valve replacement by using76native GK bileaflet mechanical heartvalves. The mitral valve replacement were performed in18patients,aortic valvereplacement in9patients,tricuspid valve replacement in2patients, double valvereplacement in22patients and triple heart valve replacement in2patients respectively.Follow-up was carried on and extended through three monthes.Results:None died from surgery. The hospital mortality of valvular surgery was5.76%(3/52). The reason of death involved in low cardiac output syndrome andventricular arrhythmia for2cases. Another was multiple organ dysfunction or failure.The hospital mortality, Cardiopulmonary bypass time, Aortic cross clamping time,Blood transfusion, ICU stay, hospital stay after operation and ventilator assistance werea little higher in the group than those other group without Native GK bileafletmechanical heart valve. The survivors have no complications obviously. Three monthesfollowing-up after operation was carried on in49patients, but3patients losing contact.46patients responsed to visit out-patient clinic and took the Ultrasoundcardiogram,Electrocardiogram and Chest x-ray. Results revealed all patients under no valve-relatedcomplications more than3monthes after operation.Ultrasoundcardiogram showed thetransvalvular pressure difference of postopration for only aortic valve replacementobviously degraded compared with that of preoperation(50.00±5.050mmHgVS.24.44±4.927mmHg,t=15.249,p=0.000) and the transvalvular pressure differenceof postopration for only mitral valve replacement also obviously degraded comparedwith that of preoperation(18.433±4.081mmHg VS.3.983±2.183mmHg,t=16.708,p=0.000).The velocity of transvalvular blood flow was not detected increasing postoperation. The LVEF increased (63.67±10.001ï¼…VS.67.44±6.235,t=-5.605,P=0.000) in all patients compared with that of preoperation. The chest x-ray filmshowed that Cardiothoracic ratio degraded(0.615±0.0961VS.0.5065±0.04067,t=11.019,P=0.000) compared with that of preoperation. In37(37ï¼46)ofsurvivors heart functional performance had improved to New York Heart Assoeiationclass I and9(9/46) had improved to New York Heart Assoeiation classâ…¡.Conclusion The homemade GK bileaflet prosthesis heart valve could be taken as anideal alternative for native heart valve for its safety, reliability,high quality and excellenthemodynamie properties. But most of this group patients had longer history and missedthe optimal time for treatment as the economic reason. The function of the patients’heartand the other body organs aggravating brought surgical treatment and postoperative carethe difficulities.The hospital mortality could be a little higher. |
