Objects:This study aimed to identify the correlation between clinical manifestation, prognosis of patients with dermatomyositis/polymyositis and anti melanoma differentiation associated gene-5(MDA5) autoantibody by ELISA. Meanwhile this study examined for their titer in course of disease to identify the relationship between titer of antibody and condition.Methods:Sera from65patients with dermatomyositis/polymyositis were screened for anti-MDA5antibody using ELISA, to compare the clinical manifestations of positive patients with negative group, meanwhile to compare the titer of anti-MDA5antibody from different groups before therapy, and to identify the association between the titer of anti-MDA5antibody and the clinical course.Results:(1) The detection rate of anti-MDA5antibody was high in dermatomyositis.(2) The patients with positive anti-MDA5antibody developed significantly more skin ulcerations and interstitial lung disease (P<0.05) than those without the antibody, while the detection rate of antinuclear antibodies and muscle enzymes were detected significantly lower in positive group (P<0.05).(3) The titer of anti-MDA5antibody in dermatomyositis with interstitial lung disease (P=0.000) and refractory dermatomyositis (P=0.001) before treatment was higher.(4) The association between the titer of anti-MDA5antibody and the clinical course was not confirmed.Conclusions:(1) Anti-MDA5autoantibody is specifically detected in dermatomyositis.(2) The patients with positive anti-MDA5antibody usually have typical skin rash and lower muscle enzymes.(3) The patients with positive anti-MDA5antibody are usually complicated with interstitial lung disease, which are poor prognosis.
