Background: The present study shows that the most importantcause of mortality in patients with malignant tumor is the tumor recurr-ence and distant metastasis,but for distant metastasis of tumor,it is not apure, simple and random process.As is known to all,each kind of distantmetastasis of malignant tumor has its specific transfer of organs, and afterradical surgery in patients with malignant tumor,almost all of the patientsdied of tumor local recurrence and distant metastasis, postoperative5-ye-ar survival rate was less than30%.For the early diagnosis of gastric can-cer patients with distant metastasis is still the problem of gastric cancertreatment in the field of research hot spot. Molecular biology characteri-stics of tumor cells,adhesion molecules and various cytokines plays avital role in the distant metastasis of gastric cancer.Recent studies haveshown that adhesion molecules E-selectin element and vascular endothe-lial cells are expressed on cancer cells and that choice more can promotethe local invasion and distant metastasis of malignant tumor,and adhesionmolecule plays an important role in gastric cancer patients with distantmetastasis.Objective: Through the detection of adhesion molecules E-selectinexpression in gastric cancer as we choose,discussing the relationship between the gastric cancer lymph node metastases and the celiac plantingtransfer,to further reveal the role and significance in lymphatic metastasis,and the process of peritoneal metastasis in the stomach tumor.Methods: We selected patient documents which suffered from theradical gastrectomy in the Jilin university affiliated NO.2hospital fromJun-2011to Jun-2013,and randomly selected53wax block samples of thepatients with advanced gastric cancer,which were all received postoperat-ive pathological diagnosis of peritoneal metastasis; We randomly selected50cases as control group which have not been found peritoneal metastasis in preoperative examination and intra operative probe.All cases rando-mly selected are not receptive to new adjuvant chemotherapy,targetedtumor biological therapy or genetic therapy,etc.All specimens were fixedwith formalin routinely,we identified the qualitative with the experiment ofimmunohisto chemical method after the paraffin embedding,to analysisthe relationship between the expression of the E-selectin and clinicalpathological parameters.Results: The expression of the E-selectin in the patients with theperitoneal metastasis and the patients without the peritoneal metastasis ofstomach cancer:53documents with the peritoneal metastasis expressedE-selectin in blood vessel endothelium,and46documents of them werepostive,and the positive rate was86.79%.50documents without the perit-oneal metastasis expressed E-selectin in blood vessel endothelium,and9 documents of them were postive,and the positive rate was18%.In cance-rous tissue vascular endothelial,E-selectin mainly expressed on the mem-brane of endothelial cell,there were significant difference between thetwo (p<0.05) groups,moreover, E-selectin also expressed on the membr-ane of cancer cell.E-selectin expression in the cytoplasm of gastric cancer cell:53casesof the patients wih the distant metastasis,gastric cancer metastases cellsexpressed E-selectin,6cases of them were postive,and the positive ratewas11.32%.There were4cases positively with E-selectin expressed onprimary focal cells,positive rate was7.54%.50cases of gastric cancerwithout distant metastasis,there was no positive case with E-selectinexpressed on primary focal cells,there was no significant difference betw-een them (p>0.05).53cases of gastric cancer with distant metastasis,there were32positive cases in the male group (32/53)and21positive cases in femalegroup(21/53).there were19positive cases among of24patients who wereyounger than55years old.there were27positive cases among of29patients who were older than55years old. According to the histologicaldifferentiation,there were5cases with high differentiation adenocarcino-ma(5/8),there were25cases poorly differentiated adenocarcinoma(25/27);there were12cases signet ring cell carcinoma (12/13) and there were4cases mucous adenocarcinoma respectively(4/5),there was no significant difference among groups (P>0.05).53cases of gastric cancer with peritoneal metastasis cases,the posi-tive rate of the cases expressed with infiltration depth in T2,T3,T4were25%、72.7%and25%respectively;the positive rate of the cases expressedwith cancer lymph node metastasis N1、N2and N3were62.5%,82.3%and96.4%respectively;here was no significant difference among groups(P>0.05).Conclusion:1、E-selectin expresses on the capillary endothelial cells in greateromentum metastases of gastric cancer,and E-selectin expression on.2、metastases in vascular endothelial cells is higher than withoutthat of distant metastases.2、As clinical stage increasing, in the group with the far greateromentum metastasis group,the expression of the E-selectin on capillaryendothelial cells in metastases also increases gradually.3、with the progress of the disease,the expression of E-selectin alsopresents a rising trend,so it is possible that the E-selectin is an importantmolecular biology markers of peritoneal metastasis of gastric cancer.
