Retrospective Comparison Of Humeral Head Replacement For The Treatment Of Senile Proximal Humeral Fractures

Objective:By comparing the treatment with shoulder arthroplasty and with plateinternal fixation (locking plate,anatomic plate) on elder proximal humeruspatients voith osteoporosis.To evaluate the clinical effiaucy of shoulder arthroplasty.Methods:Collected well followed57cases of elder proximal humerus aftersurgery in ((male18,femd39,age65-104,average age79), from mar2011tomar2013.Among the36cases fracture occurred in right,the other21casesoccurred in left.And32cases of the four-part fractures,25cases of ther arethree-part fracture.Causes of the fracture:13cases by traffic acident,39cases byfall,5cases by the other cause.The patients were olivided into shoulderarthroplasty group(22cases,average age83.90) and plate internal fixation group(35cases,average age76.06,among the23cases by locking plate and12cases byanatomic plate).Reverded the operation, bleeding and the shoulder function.Dothe statistical analysis by SPSS18.0.Results:The operation duration of shoulder arthroplasty group is significantshorter than plat internal fixation group (P=0.042).The bleeding of shoulderarthroplasty group is significant less than plate internal fixation group(P=.0.045).The fanction of shoulder arthroplasty is significant belter than plateinternal fixation group (P=0.039).Conclusions:Most aged people are with different degrees of osteoporosis.Oncethe proximal humerus fracturs occurred,the fractures are mostly three or four-partfracture.To treat the fracture in the flexible shoulder joint, it requirs not onlyrelieving pain,but also good function.Shoulder arthroplasty restored the integrityand stahitity of shoulder.After shoulder functional exercises,the patient will gainthe maximum shoulder function revorery.Above all,we prefer to shoulderarthroplasty on the elder three or four-part proximal humerus fractures patientswith osteoporosis.
