Clinical Observations On The Efficacy Of Shenyanbao Capsules In Treating Type2Diabetic Nephropathy Stageâ…¢blood Stasis Syndrome

Objective:Shenyanbao capsules(xuefuzhuyutang capsules—preparation of The211th hospital of PLA)has the pharmacological action of a ctivating circulation to remove blood stasis and tonifying kidney and dispelling turbidity action.This subject focuses on the effic acy of Shenyanbao capsules in treating patients with type2dia betic nephropathy stage Ⅲ blood stasis syndrome and its influ ence on total cholesterol(TC),triglyceride(TG),fasting blood gluc ose(FBG),postprandial blood glucose(PBG),haemoglobinAl c(HbAl c),fibrinogen(FIB),D-dimer(DD),blood rheology and24h urinates the micro abumin (UMA),C-reactive protein(CRP),blood and urin e β2-microglobulin(β2-MG).Thus confirm the efficacy of Shenyan bao capsules in treating patients with type2diabetic nephropat hy stage III blood stasis syndrome.Futhermore investigate the ro le of shenyanbao capsules in prevention and treatment of early diabetic nephropathy and provide theoretical and clinical basis f or promoting blood circulation and removing blood stasis in tre ating early diabetic nephropathy.Methods:60patients with type2diabetic nephropathy stage III blood stasis syndrome are randomly divided into two groups:shenyanb ao capsules group(n=30),and Control group(n=30).The two groups are given the basic treatment of Western Medicine(including di abetic nephropathy health education,given the high quality low p rotein,low salt,low fat,diabetes diet,control blood sugar,blood pr essure,blood lipids and other conventional treatment)and benazep ril hydrochloride tablets(Lotensin)10mg,oral,once a day.On this b asis,shenyanbao capsules group uses shenyanbao capsules,6caps ules,oral,three times a day.The course of treatment is8weeks.O bserve changes of lipids(TC,TG),glucose(FBG,PBG,HbAl c),fibrino gen(FIB),D-dimer(DD), blood rheology and24h urinates the micr o abumin(UMA),C-reactive protein(CRP), blood and urine β2-mic roglobulin(β2-MG)in all patients before and after treatment.Results:1.Comparing two groups of patients’TC,TG,PBG and HbAlc b efore and after treatment, the difference is not statistically sign ificant(P＞0.05).2.Comparing two groups of patients’overall clinical efficacy, the difference is statistically significant(P＞0.05).The effect of s henyanbao capsules group(total effective rate:83.33%)is better th an the control group(total effective rate:63.33%).3. Comparing two groups of patients’ FIB,DD before and after treatment, two groups of patients’ FIB,DD, are significantly lo wer(P＜0.01),but shenyanbao capsules group is better than the co ntrol group and the difference is statistically significant(P＜0.05).4. Comparing two groups of patients’whole blood viscosity,pl asma viscosity and hematocrit before and after treatment,two gr oups of patients’whole blood viscosity,plasma viscosity and he matocrit are significantly lower(P＜0.01),but shenyanbao capsules group is better than the control group and the difference is st atistically significant(P＜0.05).5. Comparing two groups of patients’24h urinates the micro abumin(UMA),two groups of patients’24h urinates the micro ab umin(UMA)are significantly lower(P＜0.01),but shenyanbao capsul es group is better than the control group and the difference is statistically significant(P＜0.05).6. Comparing two groups of patients’CRP before and after tr eatment,two groups of patients’CRP are significantly lower(P＜0. 01),but shenyanbao capsules group is better than the control gro up and the difference is statistically significant(P＜0.05).7. Comparing two groups of patients’blood,urinary β2-MG be fore and after treatment,two groups of patients’blood,urinary β2-MG are significantly lower(P＜0.01),but shenyanbao capsules gr oup is better than the control group and the difference is statis tically significant(P<0.05).Conclusions:1.Shenyanbao capsules can reduce FIB,DD, whole blood viscos ity,plasma viscosity,and hematocrit of patients with type2diabe tic nephropathy stage III blood stasis syndrome and the extent o f the patient’s blood stasis and slow the progression of DN.May be this is one of the mechanisms that shenyanbao capsules prev ent and treat early DN.2.Shenyanbao capsules can reduce24h urinates the micro abu min(UMA),CRP,blood and urine β2-MG of patients with type2d iabetic nephropathy stage III blood stasis,and improve the micro-inflammatory state of patients’kidneys. Thereby shenyanbao ca psules can prevent and treat DN. Maybe this is another mechani sm that shenyanbao capsules prevent and treat early DN. In sho rt,Shenyanbao capsules can prevent and treat early DN with som e clinical applications.
