Objective To quantitative assessment LV(left ventricular) three dimensional srainof CHF(chronic heart failure) patients by three dimensional speckle trackingimaging.Attempting to evaluate heart functiong by use the LV three dimensional strainparameters.Methods30healthy adults and67patients with chronic heart failure were enrolledin this study, measured basic echocardiography parameters,the full-volume imagines wererecorded and then were analyzed by on-line3-DT analysis software, storing theimages,using on machine software to analyze the parameters of the LV three dimensionalstrain by3D-STI, measured plasma BNP. And then:1. Analyzed the differences of LVstrain parameters between the CHF groupand the healthy group;2.Analyzed therelationship between LV srain and basic echocardiography systolic function parameters.3.Analyzed the differences of the relationship between LV global systolic peak srainparameters、basic echocardiography systolic function parameters and plasma BNP level,and its value of prognosis.Results1.The LV strain parameters was decreased in the CHF group, comparedwith the healthy group,there was significant difference between two groups;2. Left ventricle global peak systolic strain parameters were positively correlatedwith LVEF (r=0.515; r=0.739;r=0.728;r=0.726;P value <0.05);left ventricle globalcircular、radial、area peak syatolic strain parameters were negatively correlated withLVEDV (r=-0.442; r=-0.406;r=-0.425,P value <0.05);left ventricle global longitudinal peak syatolic strain parameters were negatively correlated with LVEDV (r=-0.263,Pvalue>0.05);3. When the plasma BNP level is super100pg/ml,the effection of left ventricularglobal longitudinal、 circular peak systolic strain parameters is higher than otherparameters;the area of left ventricular global longitudinal systolic strain under ROC curvesis0.938,has significance in diagnosis.Conclusion1.The difference of the LV strain parameters between the CHF group and the healthygroup is significant, is a reproducible;2.LV strain is valuable for the assessment of cardiac function of CHF patients;3.Among LV strain and its4constituent parameters, the decrease of longititular strainand circular strain parameters could reflect CHF patients desease levels,were significantlycorrelated with BNP levels;and Longititular strain parameters is a sensitive critia to earlyHF,has higher value.
