Objective: Discussion of adult femoral neck fracture with several cannulated screws internal fixationfor high risk factors for avascular necrosis of femoral head, provide reference for clinicaldecision-making.Methods: retrospective analysis from January1,2001to December31,2010during the cannulatedscrew fixation for the treatment of adult patients with femoral neck fracture, for age, sex, fracture type(Garden type), quality of reduction (Garden index), time from injury to operation interval, the way ofreduction (open or closed reduction), partial weight bearing time after operation, fixation removal,8factors were statistically analyzed, selected factors are statistically significant differences, and then allof the single factor by using multivariate logistic regression analysis of the corresponding strength ofassociation.Results: there were185cases of complete follow-up data, were followed up for3~10years (average5.5years), including28cases of avascular necrosis of the femoral head necrosis rate,15.14,independent determinants for avascular necrosis of the femoral head were fracture type (OR=2.882P=0.010), fracture reduction quality (OR=2.563P=0.017), time from injury to operation (OR=1.607P=0.021), partial weight-bearing time after operation (OR=1.429P=0.031).Conclusion: hollow pressure screws to treat femoral neck fracture is the main complication ofavascular necrosis of the femoral head, fracture type, fracture reduction quality, time from injury tooperation interval, partial weight bearing time after operation were the main risk factors of avascularnecrosis of the femoral head. The quality of reduction, time from injury to operation interval, partialweight bearing time after operation is controllable factors, early operation fracture reduction fixation,improving the quality of fracture reduction, internal fixation after March began to partial weight bearing can reduce the incidence of avascular necrosis of the femoral. Existing data show that the hollowcompression screw internal fixation of overall efficacy in the treatment of femoral neck fractures,femoral head necrosis rate is low, the operation is simple, small trauma, for the general condition isgood, a longer life expectancy, bone quality is still good nondisplaced or minimally displaced femoralneck fractures in elderly patients, and younger age of femur neck fractures, suggest the preferredinternal fixation treatment.
