| Purpose To investigate effect of neoadjuvant chemotherapy on surgical risk,complications, postoperative quality of life and prognosis of the advanced gastric cancer.Methods34advanced gastric patients hospitalized in Qilu hospital from2011-9to2013-9were retrospectively analyzed and followed up. All the patients were divided into the non-neoadjuvant chemotherapy (NNAC) group and the neoadjuvant chemotherapy (NAC) group. Preoperative chemotherapy was carried on the NAC group for2-4cycles and the regimen was oxaliplatin plusgimeraciland oteracil porassium capsules.Radical gastrectomy was carried2weeks after the chemotherapy. Operations were carried in the NNAC group without neoadjuvant treatment. The average operation time, the average amount of bleeding, perioperative period complications (including incision infection,anastomotic fistula and so on), thelength of hospital stay and the recurrence rate were compared between two groups. Clinical data including body weight, pathological phase, postoperative quality of life and laboratory examination results were followed up. Pathologic stage adopted the American cancer joint committee (AJCC)stomach TNM staging (2010seventh edition).Results43patients who diagnosed to be advanced gastric cancer in our center are collected, of which23males and14females, aged36to80years old. Overall response rate was58.8%(10/17), the operations were carried out smoothly. The analysis shows that operative bleeding and operative time had correlation with neoadjuvant chemotherapy(P<0.05). Perioperative complications, clearing time, postoperative hospitalization time, relapse of cancer in3and6months had no correlation with neoadjuvant chemotherapy(P>0.05). Conclusion Neoadjuvant chemotherapy had no influence in the surgical risk and could improve the prognosis of advanced gastric cancer patients through decreasing the clinicopathological stage. Refractory result to the neoadjuvant chemotherapy could spare unnecessary surgery. |
