| Objective:To assess the condition of the continuous ambulatory peritoneal dialysispatients who are in the peritoneal dialysis center of Second Affiliated Hospital of SoochowUniversity,Study the reason of dropout,analyze the PD patients’ clinical characteristics,thus to guide the improvement of the quality of life and prognosis in patients withperitoneal dialysis.Methods: Select the patients in the peritoneal dialysis center of Second AffiliatedHospital of Soochow University who were done the percutaneous peritoneal dialysiscatheter insertion and started peritoneal dialysis and with a long-term follow-up fromDecember30,1999to1999on May31.Collect the patients’ clinical data,record thepatient’s gender,age,primary disease,the condition of outcome,the reason of outcome,the date of percutaneous peritoneal dialysis catheter insertion,death date,peritonealdialysis age,the times of peritonitis and so on.Outcome is divided into: remained onperitoneal dialysis,hemodialysis,peritoneal dialysis combined with hemodialysis,death,kidney transplantation,and lost to follow-up,and the cause of turn out of peritonealdialysis is death,turn to blood dialysis,peritoneal dialysis combined with hemodialysis,kidney transplantation,and lost to follow-up.Analysis the patients of our center who weredone the percutaneous peritoneal dialysis catheter insertion,started peritoneal dialysis andfollow-up more than1year from January2004to December2011.To calculate theseperitoneal dialysis patients1year,2years,3years survival rate,technical failure rate,mortality rate.Analysis the influence of the age,the primary disease,and the occurrenceof peritonitis in dropout rates. Statistical analysis was performed using SPSS forWindows,version17.0(SPSS Inc,Chicago,IL,USA),measurement data usingx±sto description,enumeration data expressed as case or percentage,the comparison of samplemean using t-test,the comparison of enumeration data using X2test,and P <0.05was considered statistical significance.Results: There were462patients done the percutaneous peritoneal dialysis catheterinsertion,started peritoneal dialysis and with a long-term follow-up since On December30,1999to May31,2013in the nephrology department of the Second Affiliated Hospitalof Soochow University.The patients remained on PD in290case,and172cases dropoutin peritoneal dialysis,of the172cases patients77cases were females (44.8%),totaldropout rates was37.2%,the mean peritoneal dialysis duration was (20.73±18.949)months.Removal of lost for follow-up in14cases,renal function recovery in2cases,theprimary diseases for end stage renal disease of remaining446was chronicglomerulonephritis,(216,48.4%),diabetic nephropathy (83,18.6%) and hypertensiverenal impairment (63,14.1%),polycystic kidney (28,6.3%),in which chronicglomerulonephritis was the most important primary disease.Dropout causes including diedin93cases(54.1%),changed to hemodialysis in42cases(24.4%),combined withhemodialysis in7cases (4.1%),treated with kidneytransplant in14cases(8.1%),renalfunction recovered in2cases (1.2%),and lost for follow-up in14cases(8.1%).The mainreasons of death was cardiovascular events (34,36.6%),follow by cerebrovascular events(19,20.4%),and infection (9,9.7%),malnutrition (9,9.7%).The main causes ofturn to hemodialysis and peritoneal dialysis combined with hemodialysis were included ininfection (11,22.4%), insufficient peritoneal dialysis(10,20.4%), functionalabnormalities of peritoneal catheter (5,10.2%),Heart function failure (4,8.2%),can’ttolerate peritoneal dialysis (3,6.1%).Conclusion: In our peritoneal dialysis center,outcome in peritoneal dialysis patientsis divided into: remained on peritoneal dialysis,hemodialysis,peritoneal dialysis combinedwith hemodialysis,death,kidney transplantation,and lost to follow-up the main reason ofdropout from peritoneal dialysis was death, death, fo1lowed by the change tohemodialysis.The main reason of leading causes of death is cardiovascular events,infection and malnutrition,the main reason of change to hemodialysis and peritoneal dialysis combined with hemodialysis was infection,insufficient peritoneal dialysis,andfunctional abnormalities of peritoneal catheter.... |
