purposePreoperatively by2d-DSA,3d-DSA for intracranial aneurysms interventional treatment process, detailed understanding of anatomical characteristics of intracranial aneurysms and the shape of aneurysms, and the relationship between parent artery and aneurysm, aimed at reasonable and effective use of digital flat panel detector DSA imaging mode, an accurate diagnosis and evaluation of intracranial aneurysms, select the appropriate treatment and guide treatment.Materials and methods77patients suffered from intracranial aneurysm in our hospital from May2010to Aprial2013were retrospectively analyzed,48cases of male and29cases female. Patients aged24to76, with an average age of54.6±5.78. Clinical with intermittent or sudden headache, unilateral oculomotor nerve paralysis, such as performance.54cases were confirmed by imaging examination preoperatively subarachnoid hemorrhage.All of the patients were examined by2D-DSA and rotational DSA, Data from rotational DSA were transmitted to the3D workstation for processing. Try to use post-processing images for diagnosis and evaluation of intracranial aneurysms; In determining for patients with endovascular embolization of aneurysms, reasonable choose embolism method and work position; As to precise intubation,data from examinations were used to reduce the difficulty of intubation. And how to choose appropriate reduction shadow map to determine the degree of embolization; Review after embolization, the use of imaging what mode again and decide whether to have clear display without recurrence of embolization. All patients with confirmed after endovascular embolization treatment, as a reference standard. By two senior titles of nerve radiologists independently of3d-DA all sorts of commonly used models to evaluate image and rotational DSA image, finally it is concluded that agreement.The resultsAngiography of intracranial aneurysms, according to the details of this study compared the2d-DSA images, rotational DSA images and3d reconstruction images show about the relation between space of aneurysm and its parent artery, tumor neck display, the shape of aneurysms. For the display of the neck of aneurysm,rotational DSA images had higher score than2d-DSA, the difference was not statistically significant (P>0.05). And the pattern of various commonly used3d-DA image were better than rotational DSA,which had statistical significance (P<0.05).For grading the aneurysm shape, the highest score is the SSD images and VRD images, the lowest score is conventional2d DSA images.which was consistent with the results of previous literature research.For the aneurysm shape showed2d-DSA and rotational DSA no significant difference (P)0.05). And rotational DSA image than3d-DA images of various common patterns, the difference was statistically significant (P<0.05).3d-DA image between all kinds of commonly used model, showed no statistically significant difference (P>0.05).For the spatial relationship of aneurysm and its parent artery, the pattern of various commonly used3d-DA image were better than rotational DSA(2d-DSA are graphic image and could not be compared here), the difference was statistically significant (P<0.05). And rotational DSA image is clearly various commonly used mode of3d-DA image, the difference was statistically significant (P<0.05).3d-DA image between all kinds of commonly used model, showed no statistically significant difference (P>0.05).conclusion3d-DSA could show aneurysms, umor neck,the spatial relationship between tumor neck and its parent artery more clearly, provides the optimal work point of view, and could significantly reduce the dosage of contrast agent and X-ray radiation dose and improve the safety of radiological protection. According to diagnosis, interventional embolization of intracranial aneurysms and the different situation of postoperative review, reasonable and effective application of3d digital flat panel detector DSA imaging mode, to improve the accurate diagnostic rate of intracranial aneurysms, improve the technology level of interventional embolization of intracranial aneurysms, reduce complications and lower recurrence rate.
