Analysis Of Ultrasonographic Characteristics For Benign And Malignant Thyroid Nodules Using Logistic Regression

Objective:By establishing a Logistic regression model of thyroid ultrasonography in differential diagnosis of benign and malignant thyroid nodules in the meaning of summary the characteristics of benign and malignant thyroid nodules by using the ultrasonography.Methods:Confirmed by pathology of thyroid nodules in115cases of a total of173nodules, preoperative record of gray scale and color Doppler were made to score the Ultrasonographic features of thyroid nodules using Logistic regression analysis to filter out differential diagnosis of benign and malignant thyroid nodules in the independent feature index, draw a ROC curve to evaluate forecasting capability for Logistic regression modes.Results:①The significant characters to differentiate malignant from benign were theShape, echostucture, taller-than-wide shape, margin, halo, calcification, blood, lymph nodes of neck of thyroid nodules;②In this study, malignant nodules accounted for42, of which36nodule is solid nodules, low echo solid nodules in28(77.8%), with a solid hypoechoic nodules (11.1%), four high echo4solid nodules (11.1%); Benign nodules accounted for131, including53solid nodules, hyperechoic solid nodules in17(32.1%), with a solid hypoechoic nodules in19(35.8%), low echo solid nodules in17(32.1%);③Five ultrasonographic features were finally entering the Logistic model which were shape, echostucture, taller-than-wide shape, halo, calcification.The model was:logit (p)=-17.103+1.281X1+2.010X2+3.182X4+1.026X5+1.193X6. The percentage correct of the model prediction was90.7%;④Malignant=1/1+e-z, Z=bo+b1X1+......+bpxp,(e=2.718), when the irregular shape, low echo, aspect ratio>1, silent halo surrounding, grains of sand "calcification, malignant possibility is0.99, when shape rules, high echo, aspect ratio<1, complete with acoustic halo around, without calcification, malignant possibility of0.02; ⑤The area under the ROC curve was0.957, Standard error is0.046, P<0.001,95%confidence interval (0.732,0.911).To illustrate the model’s fitting better, in the prediction of benign and malignant thyroid has remarkable significance.Conclusions:Five ultrasonographic features and radioisotope scanning result were finally entered into the Logistic model; they were shape, margin, halo, calcification, blood, lymph nodes of neck.
