Objective Analysis of bone cement perfusion percutaneous balloon dilatation ofkyphoplasty treatment of osteoporotic thoracolumbar vertebral compression fracturesaffect the clinical efficacy and complications, For clinicians to implement percutaneousballoon kyphoplasty expansion choose a safe and effective amount of bone cementperfusion provide references. and changes in the vertebral and middle of vertebralheight difference, cement leakage rate, the incidence of fractures, analysis of bonecement injection volume and VAS scores changed vertebral height restoration, cementleakage and recurrence-related fractures, Investigate the effect of the amount of bonecement injected percutaneous balloon dilatation of kyphoplasty treatment ofosteoporotic vertebral compression fractures clinical efficacy and complications. Methods Retrospective analysis of the First Affiliated Hospital of Anhui MedicalUniversity in June2012, June2013passing balloon dilation kyphoplasty treatment of208cases of thoracolumbar osteoporotic vertebral compression fracture patients (257vertebrae), met the inclusion criteria and received follow-up of64patients. Dependingon the dose infusion of bone cement will be divided into: low-dose group (0.5mL~3mL)16cases; dose group (3.1mL~6mL)35cases; high-dose group (greater than6mL)13cases. Observations index before and after surgery VAS score, vertebral Andmiddle vertebral height, cement leakage, recurrent fractures. All data were statisticallyprocessed, The three groups were compared preoperative and postoperative VAS score, and middle vertebral height, group among the three groups before and after surgery tochange the difference between VAS score, and middle vertebral height differencechanges, cement leakage rate, then the incidence of fractures. Analysis of bone cement injection volume and VAS scores changed vertebral height restoration, cement leakageand recurrence-related fractures, Investigate the effect of the amount of bone cementinjected percutaneous balloon dilatation of kyphoplasty treatment of osteoporoticvertebral compression fractures clinical efficacy and complications. Results Case studies were successful surgery. One case of male T12vertebralfractures in patients with severe heart rate, difficulty breathing, blood pressure and otherreactions at the injection of bone cement process, immediately stop bone cementperfusion, give oxygen, boost and other symptomatic treatment vital signs returned tonormal, after ann returned to the ward.9patients with vertebral bone cement leakage,no spinal cord, neurological symptoms, All patients were followed up for6to12months, found eight cases of recurrent fractures. All patients significantly reduced pain,VAS score from preoperative7.64±1.36to postoperative2.28±1.21, The differencewas statistically significant (P <0.05). Compare the difference between the three groupsVAS scores before and after surgery, the difference was not statistically significant (P>0.05). All patients average anterior vertebral height from preoperative14.96±2.60mm rose after19.68±2.59mm, The difference was statistically significant (P <0.05).The average height of the middle vertebral of the preoperative15.98±2.20mm roseafter20.42±2.55mm, the difference was statistically significant (P <0.05). Compare vertebral and middle Vertebral average height difference among three groups before andafter surgery, low-dose group and middle dose group and high dose group, thedifference was statistically significant (P <0.05), middle dose group and high dosegroup, the difference was not statistically significant (P>0.05).9cases of leakage ofcases in which the low-dose group1patients, the dose of four cases, the high-dosegroup4Cases, three groups of bone cement leakage rate was not statistically significant(P>0.05). However, the leakage rate of the high-dose group (30.7%) is much higherthan the low-dose group (6.3%) and medium-dose group (11.4%).8cases of recurrentfractures, there are five cases of re-fracture pain, adverse events,3cases of line againPKP treatment, two cases of conservative treatment, pain relief, there are three cases noobvious symptoms, follow-up observation. Among the three groups fracture recurrencerate was not statistically significant (P>0.05), but the incidence of leakage of high-dosegroup (30.7%) is much higher than the low-dose group (6.3%) and medium-dose group(8.6%).Conclusion1. PKP surgery can effectively relieve pain, partially restored vertebralheight, is the treatment of thoracolumbar osteoporotic vertebral compression fracturessafe, effective, minimally invasive new technologies, with good prospects for clinicalapplication.2. the degree of bone cement to ease the pain of the injection volume was nosignificant correlation.3. in a certain range of vertebral injuries front, middle height and bone cement volumecorrelated.4. the high-dose group, cement leakage rate, then a higher incidence of fracturesrequire attention.
