| Purpose : To explore the mechanism of reduction of nephrotoxicity of Bao Yuan PaiDu Wan and the clinical efficacy of Bao Yuan Pai Du Wan to different syndromes ofcisplatin-induced nephrotoxicity. It aimed at the reducing the cisplatin-inducednephrotoxicity, ensuring the smooth progression of combined chemotherapy containingcisplatin without affecting antitumor activity of cisplatin.Methods: This article includes Chinese literature research and clinical research in twoparts. The first part described the literature of modem medicine and traditional Chinesemedicine on cisplatin nephrotoxicity, including name, etiology and pathogenesis, itslocation and treatment, also including the research of mechanism of prescription of BaoYuan Pai Du Wan and the pharmacology of all compositions of Bao Yuan Pai Du Wan.The second part is clinical research. 120 cancer patients were randomly divided into twogroups, one was the observer group (cisplatin+Bao Yuan Pai Du Wan) and the other onewas the control group (cisplatin), each of them contained 60 patients. According to thestandard agreement, all cases were further divided into four groups, the first one was groupof heavy dampness caused by spleen-deficiency, second one was group of deficiency ofkidney-qi, third one was group of yin-deficiency of liver and kidney and last one was groupof yang-deficiency of spleen and kidney. 100mg/m2 Cisplatin (total doses in the treatment)would be included in chemotherapy of patients of both groups. Patients in the observergroup took Bao Yuan Pai Du Wan 10 tablets three times daily in the first day afterchemotherapy for l0 days. After that we examined theα1 micro globulin (α1-MG), urinaryN-hexanoyl-β-D-amino-glucosaminidase (NAG) of patients of two groups before thechemotherapy, on the 1st day and 10th day after chemotherapy by using immunenepherlmetry and chemocolorimetry. Besides, we also examined the size of the lesionsbefore and after chemotherapy by using computed tomography (CT).Results:1. The urinaryα1 micro globulin (α1-MG) and urinary N-hexanoyl-β-D-amino-glucosaminidase (NAG) of two groups significantly increased when compared with thatbefore chemotherapy, and after taking Bao Yuan Pai Du Wan, the urinaryα1 micro globulin(α1-MG) and urinary N-hexanoyl-β-D-amino-glucosaminidase (NAG) of observer groupsignificantly decreased when compared with control group (P<0.01). 2.The degree of renal impairment of both groups had no statistical significance(P>0.05) on the 1st day after chemotherapy. On the 10th day after chemotherapy, the degreeof renal impairment of observer group was lower than that of control group, and thedifference of degree of renal impairment between two groups was statistically significant(P<0.05).3.The urinaryα1 micro globulin (α1-MG) of the group of heavy dampness caused byspleen-deficiency, deficiency of kidney-qi, yin-deficiency of liver and kidney andyang-deficiency of spleen and kidney decreased in varying degrees, and the group ofdeficiency of kidney-qi was the most significant one within the four subgroups of theobserver group, significantly improved rate was 88.24%. When compared with the otherthree subgroups, the difference was statistically significant (P<0.05).4. The clinical efficacy and rate of progression of the observer group was 32.35%and17.65%respectively, and that of control group was 27.27%and 21.21%respectively. Thedifference between two group had no statistical significance (P>0.05).Conclusion : Bao Yuan Pai Du Wan can reduce urinaryα1-MG, NAG. It can preventand treat cisplatin-induced renal damage. But the effect is the best in the group ofdeficiency of kidney-qi. |
