| [Objective]Based on the reference of research at home and abroad, this study aimed at home movement dysfunction recovery in patients with stroke using the method of questionnaire survey, the methods of literature analysis, Delphi method, form the recovery movement dysfunction in patients with stroke screening the applicability of the home rehabilitation technology experts write questionnaire, applicable to home rehabilitation technology under the condition of security, competency and applicability of screening; And through the analytic hierarchy process (AHP) to determine the weight of the above three indicators, in order to occupy the convalescence stroke patients to provide effective and practical method of rehabilitation training, appropriate to occupy the home rehabilitation technology implementation and subsequent research in community to provide the reference, to promote community rehabilitation service work at home.[Methods]This study first to recovery in patients with stroke in community rehabilitation training demand survey in Beijing, according to the survey in the questionnaire score greater than or equal to3points of entry, and combine with the literature analysis form the recovery movement dysfunction in patients with stroke home rehabilitation technology applicability of screening experts enquiry for the first draft of the questionnaire. Delphi was applied for the final home rehabilitation technology.The full score ratio, mean, standard deviation, coefficient of variation, W Kendall harmony coefficient were used for statistics statistical analysis, the result of the specialist enquiry conducted two rounds of expert enquiry, form expert enquiry for final version.According to the second round of the enquiry for using AHP to calculate weight of the three indicators.[Results]In the demand survey, the result indicates that the use of only crutches, wheelchair practice and daily life of auxiliary appliance use low scores of the two items below3points, scored the other entries in more than3points, demand levels are above58%, the level of demand in the top five in turn for fingers in the activities of practice and daily life activities ability training practice, method of position transformation method, walk, turn practice method. Through the demand survey, we summarized the18rehabilitation training content; on the basis of it’s through literature analysis to find a suitable rehabilitation technology.In the literature research, as much as possible into the appropriate rehabilitation technology, retrieval for nearly4years of domestic movement function rehabilitation of stroke with194references, through the analysis of the literature found that194references were published in the journal of36species, of which most of the published literature periodicals was followed by Chinese journal of physical medicine and rehabilitation, Chinese journal of rehabilitation medicine, rehabilitation theory and practice of China, Chinese journal of rehabilitation, traditional Chinese and western medicine combined with cardio-cerebrovascular disease, etc.73.7%of the total number of documents, related to the rehabilitation of patients with stroke degree is high; In terms of academic attention, were little changed from2011to2012, the overall trend shows that research on movement function in patients with stroke rehabilitation has higher academic attention in recent years; Quantitative research with128references, accounted for66%, other are descriptive research; There are130(67.0%) references applied statistics analysis method; All the literature, the majority of providers of rehabilitation for the rehabilitators (68%); There are130references with outcome evaluation, with the top three evaluation index respectively all kinds of rating scale, quality of life, incidence of adverse events. According to the references content and questionnaire as well as the domestic outside stroke prevention and rehabilitation guide summarizes rehabilitation technology content, the formation of the three muscle strength of a total of59items recovery movement dysfunction in patients with stroke home rehabilitation technology content.In Delphi study, after two rounds of expert enquiry, expert advice is concentrated, screening got a total of38items is suitable for the patients of home rehabilitation technology content, based on the results of the second enquiry for AHP is used to determine the weights of three indexes, according to the results of security (0.4894)> applicability (0.2921)> competency (0.2186), The results show the applicability and security of these two indicators are more important in the evaluation of home rehabilitation techniques in the practical application.[Conclusions]Rehabilitation training of stroke patients is one important way to restore the body’s functions, as well as higher demand for convalescent patients with stroke rehabilitation technology at home, so the community home rehabilitation of stroke patients should be given high priority, give full play to the role of community health care systems to promote the patients’physical and mental health, improve the quality of life of patients and reduce the burden on society and the family.At present, the home rehabilitation of stroke patients works carried out in the community is not wide, the main reason is that our country community rehabilitation service system is not perfect, lack of professional rehabilitation, community is understaffed, professional skills need to be improved, so it is urgent to strengthen community health personnel input; And based on the current situation of community rehabilitation service, form suitable for the recovering patients with stroke self-training home rehabilitation technology is one of the effective methods to help patients recover physical function, and can improve the self-care ability of patients at the same time, make their own gradually achieve the best in good condition. |
