Idiopathic membranous nephropathy is one of common pathogenic factors leads to adult nephrotic syndrome. And the detection rate of idiopathic membranous nephropathy is increasing year after year, which is accompanied by the developmentment of renal biopsy technology and the accumulation of treatment experience. At present, the pathogenesis of this disease is still unclear, the basic treatment in major Western Medicine Hospital is using the method of hormone therapy plus cyclophosphamide or the other immunosuppressive therapy, but there is larger difference between individual patients in prognosis. Either relieving symptoms or reducing relapse and the side effects of hormones, the treatment of traditional Chinese medicine have excellent clinical efficacy. But the TCM syndrome type of this disease lack of uniform standards and large sample of scientific investigation.Objective:To investigate and analysis the signs and symptoms of idiopathic membranous nephropathy patients, to explore TCM syndrome types of this disease and comprehend the pathogenesis regularity.Methods:This study is in the form of the questionnaire, including100idiopathic membranous nephropathy patients’ signs、symptoms and related laboratory data, and the patients distribute in The first hospital of Peking University, Peking Union Medical College Hospital, China-Japanese Friendship Hospital, General Hospital of Beijing Military Region, The Second Affiliated Hospital of Beijing University of Chinese Medicine. Classify the signs and symptoms based on Cluster Analysis, and explore the TCM symptom type of idiopathic membranous nephropathy.Results:According to statistics,69male patients and31female patients in100patients, the ratio of male and female is2.22:1. The average age is54.03±3.185years old in investigation, account ing for98%of all the patients is Ⅰ-Ⅱ phase of membranous nephropathy.90patients who received treatment due to edema,8patients due to urinary protein,1patient due to frequent urination and nocturia,1patient due to hypertension headache. The top five signs and symptoms are:foamy urine, fatigue, lower limb edema, nocturia and flushing. Through cluster analysis, the best result is invided into six groups, and there are21patients in the fourth group,50patients in the fifth group,25patients in the sixth group, this3groups present3TCM symptom type. Conclusion:According to the results and the analyses, idiopathic membranous nephropathy patients, especially the patients with short duration, can divided into the following three TCM symptom types:1wind-heat into the kidney;2spleen and kidney deficiency, wind and heat into kidney;3liver and kidney, wind thermal disturbance kidney. As the duration of the extension, the deficiency symptoms is increasingly, especially in spleen and kidney deficiency. The basic nosogenesis is real lead to imaginary and the mix of real and imaginary.
