Chrono-chemotherapy Of GP Regimen On The Advanced Non-small-cell Lung Cancer

【Objective】Compared chrono-chemotherapy of GP regimen withroutine-chemotherapy on the advanced NSCLC in the recent、 long-termefficacy and toxicity and also the improvement of quality of life， which aimedto explore the advantages of chrono-chemotherapy of GP regimen onadvanced NSCLC， so that individualized regimen can be selected anddeveloped．【Methods】A retrospective analysis of69cases with advancedNSCLC，who were all treated with systemic chemotherapy of GP regimen，admitted to our hospital from June2008to June2010showed that34patientswere treated by chrono-chemotherapy while35patients were treated byroutine-chemotherapy。Chrono-chemotherapy group with1000mg/m2on days1，8intavenous infusion of gemcitabine(GEM) at03：00which should befinished within30minutes， cisplatin (CDDP)25mg/m2on days1－3at15：30－16：30finished within1hour． While routine-chemotherapy groupusing the same dosage with chrono-chemotherapy but according to theconventional time10：00．Adjuvant treatment： two groups all give granisetron3mg and metoclopramide20mg to prevent tomiting0．5h beforechemotherapy in the first day， and also to give reduced glutathione1．8g toprotect the liver and dexamethasone for preventive treatment。If white bloodcells fall below3．0x109/L， recombinant human granulocyte colonystimulating factor (G-CSF) can be treated． The hydration maintainthroughout the whole process of treatment．21days for a course，at least twocourses of treatment completed to assess．【Results】①The recent curative effect： the response rate of chrono-chemotherapygroup was61．8%，and routine-chemotherapy group was37．1%(P<0．05)。 ②The long-term curative effect：median time to progression were6．9monthsand6．6months (P＞0．05) in chrono-chemotherapy group androutine-chemotherapy group，and1-yearsurvival rate were41%and36%respectively (P＞0．05)．③Toxicity： chrono-chemotherapy group was associated with less grade Ⅲ-Ⅳ neutropenia and thrombocytopenia than routine-chemotherapygroup(P<0．05)． Haematoglobin drease didn’t show any statistical differencein two groups。There was no statistically difference in nonhematologic toxicitybetween the two groups．The occurring rate of grade Ⅲ-Ⅳ nausea andvomiting of chrono-chemotherapy group and routine-chemotherapy were23．5%and48．6%(P<0．05)．○4The quality of life： the response rate of chrono-chemotherapy group was82．4%， and routine-chemotherapy group was60%(P<0．05)． Two groupsof contrast of the improvement of the quality of life was statistically different．【Conclusion】Compared with routine-chemotherapy， chrono-chemo-therapy of GP regimen can improve the recent curative effect，reduce adversereactions， so that patients are more likely to tolerance， improve the qualityof life， and most important that it can be in clinical promotion．...
