Objective:To explore the imaging features and practice value of diffusiontensor imaging (DTI) combined with proton magnetic resonance spectroscopy(1H-MRS) in early diagnosis of full-term neonatal hypoxic ischemic encephalopathy(HIE). The ratio of metabolites, ADC and FA were calculated and analyzedstatistically in the lesions of the right posterior limb of internal capsule, lenticularnucleus and the dorsal ventral lateral nucleus of thalamus.Methods:Routine MRI and2D1H-MRS、DTl were performed in20cases(patients who had been diagnosed with HIE through clinical and imagingexamination)and20healthy term newborn as a control group. NAA/Cr, Cho/Cr,LAC/Cr, GLx/Crratio,ADC and FA were calculated and analyzed statistically in thelesions of HIE(1esions in the right posterior limb of internal capsule, lenticularnucleus and the dorsal ventral lateral nucleus of thalamus) and normal white matterareas of the control group,and the change of NAA,Cho,Cr, GLx and Lac peakswere analyzed.Results:1.1H-MRS: The hight of peaks of NAA,Cho,GLx,Cr and Lac is normalin control group,and values of NAA/Cr, Cho/Cr and GLx/Cr were also normal,andthere is not a significant difference in group (P>0.05). NAA and Cho1eaks decreasedslightly in HIE lesions; the obvious Lac and GLx leaks were observed; NAA/Cr andCho/Cr decreased and there was a significant difference between HIE lesions and thecontrol group(P<0.05); and LAC/Cr and GLx/Cr increased obviously and there was asignificant difference in HIE lesions compared with the control group(P<0.05).2.DTI: FA and ADC decreased obviously in HIE lesions while they were normal in thecontrol group and there was a significant difference among them(P<0.05).3. ADCand FA were obviously negatively correlated to LAC/Cr and GLx/Cr,and positivelycorrelated to NAA/Cr,Cho/Cr in HIE lesions.And the correlation degree was mostobvious between ADC and FA with GLx/Cr.Conclusion:1)1H-MRS combined with DTI could improve accuracy of in earlydiagnosis of full-term neonatal HIE in quantitative and cerebral metabolism research respectively.2) It could indicate infant poor prognosis that FA decreased obviously inHIE lesions compared with the control group.3) ADC and FA were obviouslynegatively correlated to LAC/Cr and GLx/Cr,and positively correlated toNAA/Cr,Cho/Cr in HIE lesions.4) In assessing sensitivity and specificity of full-termneonatal HIE, FA was better than ADC; The sensitivity of GLx/Cr was higher thanLAC/Cr, but the specificity lower than it.
