The Clinic Research Of Lesion Threshold Screening Of Mirena To Treatment Adenomyosis

Objective: To observe and analysis the clinic effect of treatment adenomyosis withdifferent uterine wall thickness by using Mirena with multiple parameter, and screeningout the effective lesion threshold of Mirena when treatment adenomyosis, for the furtherpurpose of providing the basis of the tentative idea to treatment adenomyosis accordingto the indexing.Methods: Distinguishing patients of adenomyosis into mild, moderate and severe levelsaccording to single-side uterine muscle thickness (front, back and bottom of uterine) ordiameter of adenomyoma(L) by B-ultrasound.84cases patients of adenomyosis weredivided into four groups as A, B, C and D according to the different thickness of uterinewall(about5mm). The thickness of uterine muscle wall or diameter of adenomyoma infour groups are: A group (10cases): L≤25mm；B group（28cases）：25mm＜L≤30mm；C group（17cases）：30mm＜L≤35mm；D group（29cases）：L＞35mm。Mirena wereintrauterine placed within menstrual period in patients in four groups. The changes of thescales of menstrual quantity and dysmenorrhea, as well as the uterine volumes andCA125were contrastive analysis before and after Mirena used in different observationtime. Adverse reaction and complication were observed ai the same time.Result:84patients were treated with mirena,5cases had alterated operation or othertherapy methode because of uraginal ring shedding or have not clinic effect,1patientincarceration in LNG-IUS and1patient taken out of mirena. Significant changes of thescales of menstrual quantity and dysmenorrhea, the uterine volumes and CA125wereobserved in different observation time after mirena used both in group A and B (P＜0.05),while in group C, the changes were only showed in scales of menstrual quantity anddysmenorrheal (P＜0.05), although the uterine volumes and CA125dropped in somedegree, but no significant were observed in the period of this research (P＜0.05), Noobvious varieties of all observation index mentioned were found in group D(P＞0.05).Conclusion:1、Mirena have a good clinic effect of treatment adenomysis when theuterine muscle thickness or diameter of adenomyoma(L) was less than30mm. while Lbetween the30mm and35mm, the clinic effect were only showed in scales of menstrual quantity and dysmenorrheal, but the dropped of uterine volumes and CA125need toobservation longer time. Mirena have no clinic effect of treatment adenomysis while Lwas more than35mm.2、There existed a kind of selectivity for lesion size when treatment adenomyosiswith LNG-IUS, which is the lesion threshold for treatment adinomyosis effectively, andthe threshold value was the maximum diameter of uterine wall ofadenomyosis/adenomyoma was less than35mm.
