Object: To investigate the significance of serum CRP and IL-32in patients withchronic superficial gastritis, Chronic atrophic gastritis, precancerous lesion andgastric cancer for providing an important serological basis for diagnosis of earlygastric cancer.Methods:131in-patients in the Digestive Disease Department of QinghaiUniversity Affiliated Hospital from February2013to December2013were involvedin this study.27cases of chronic superficial gastritis (control group),38cases ofchronic atrophic gastritis,31cases of precancerous lesion and35cases of gastriccancer were inclued. To detect helicobacter pylori (Hp), we used14C-urea breath test(14C-UBT). The serum level of CRP was measured by nephelometry, and the serumlevel of IL-32was examined by ELISA. The results were compared among groups.Results:(1) The serum levels of CRP and IL-32in gastric cancer patients weresignificantly higher than those in precancerous lesion, chronic atrophic gastritis andcontrol group, respectively (P<0.05). The serum levels of CRP and IL-32weresignificantly higher in precancerous lesion patients than those in chronicatrophic gastritis and control group, respectively (P<0.05), however, there was nosignificant difference between atrophic gastritis group and control group (P>0.05).(2)In precancerous lesion group and gastric cancer group, the levels of CRP and IL-32were significantly higher in patients with Hp(+) than those in patients with Hp(-)(P<0.05), but there were no significant differences between patients with HP(+) andHP(-) in control group and chronic atrophic gastritis group (P>0.05).(3)It is positivecorrelation between serum IL-32and CRP in patient with precancerous lesion group(r=0.615,P<0.05). And positive correlation in gastric cancer group (r=0.775,P<0.05) as well.Conclusion:(1) The serum CRP and IL-32levels in gastric cancer group andprecancerous lesion group were significantly higher than those in control group andchronic atrophic gastritis group, respectively. This result indicates that the expression of CRP and IL-32were activated when gastric mucosa damage is becoming moreserious. The serum levels of CRP and IL-32might be the early warning indicators ofgastric cancer.(2) The CRP and IL-32levels in Hp(+) patients were significantlyhigher than those in Hp(-) patients in the groups of precancerous lesion and gastriccancer, respectively, while no significant difference in the control group and chronicatrophic gastritis group. It might be speculated from the results thatanti-inflammatory cytokines produced by the immunocytes after Hp infection couldprevent excessive inflammatory reaction. But they can not prevent inflammationinvasion and finally induce and promote the occurrence and development of gastriccancer if there is inflammatory persistently.(3) The concentrations of IL-32and CRPin precancerous lesion groups and the gastric cancer groups are positively correlated,the IL-32could promote the release of cytokines, which could further lead to thesecretion and release of CRP from hepatic cells, IL-32and CRP may be importantmediators of inflammation of gastric cancer.
