Objective By detecting the TCF21gene methylation level in the tissue and urinesamples of renal cell carcinoma to analyze the relationship and correlation betweenTCF21methylation level and clinical pathological parameters of renal cellcarcinoma,Receiver operating characteristic(ROC) curve was created by plotting thesensitivity and specificity of the renal cell carcinoma tissue specimens and the urinespecimens as an indicator for the diagnosis of renal cell carcinoma, Thus to explorethe effect and clinical significance in the early diagnosis and prognosis of renal cellcarcinoma.Method55renal cell carcinoma tissue specimens were selected as experimentalgroup and21tumor-adjacent normal kidney tissue specimens were randomly selectedas control group,while33preoperative urine specimens from renal cell carcinomapatients and15normal human urine specimens were randomly selected as a controlgroup. Quantitative detection the specimens, methylation level of TCF21bypyrosequencing.Differences in quantitative methylation values between theexperimental group and control group were assessed by the mann-whitney U-test.Therelationship between methylation levels and clinical pathological parameters such asgender,tumor stage,and grade,were evaluated using the Mann-Whitney tests.ASpearman nonparametric corrrlation test was additionally performed to compareclinical pathological parameters and methylation levels.A receiver operatorcharacteristics(ROC) curve was created by plotting the true positive rate(sensitivity) against the false-positive rate (1-specificity),and the area under the curve (AUC) wascalculated.Analysis and comparison the different methylation levels between tissuespecimens and urine specimens in diagnosing renal cell carcinoma.Result1.The TCF21gene methylation levels were significantly higher in renal cellcarcinoma than in adjacent normal kidney tissue,the differences between cancer tissueand normal tissue were statistically significant (P <0.05). The TCF21methylationlevels are closely related to age, smoking history,Fuhrman grade and clinical stage (P<0.05).There is a positively correlation between the TCF21methylation levels andage (R=0.166,P=0.002), smoking history (R=0.321,P=0.017) and Fuhrman grade(R=0.271,P=0.045) in Renal cell carcinoma.2. The TCF21gene methylation levels were significantly higher in Renal cellcarcinoma urine specimens than in normal urine specimens,the differences betweenthe Renal cell carcinoma urine specimens and normal urine specimens werestatistically significant (P <0.05); The TCF21methylation levels are closely related totumor size, Fuhrman grade and clinical stage (P <0.05).There is a positivelycorrelation between the TCF21methylation levels and tumor size (R=0.622,P=0.000), Fuhrman grade (R=0.441,P=0.010) and clinical stage (R=0.411,P=0.017)in Renal cell carcinoma.3. The ROC curve area of TCF21methylation levels in renal cell carcinoma tissue ishigher than the renal cell carcinoma urine specimens’,and the differences werestatistically significant (P <0.05). Under the critical value the sensitivity andspecificity of the tissue biopsy specimens was89%and61.9%while the sensitivityand specificity of the urine specimens was79%and100%.Conclusion TCF21is associated with the occurrence of renal cell carcinoma, The methylation levels of TCF21in renal cell carcinoma can be used as good markers forearly diagnosis of renal cell carcinoma.The TCF21methylation levels mey haveimportant effects in the development,invasion and differentiation of renal cellcarcinoma.The advantages of urine specimens include convenient to collection,noninvasive and accurate,et al. The noninvasive high flux methylation detection ofurine specimens can be used as an important supplement to tissue methylationdetection, can be used as important means for judgment of renal cell carcinomaprognosis,malignant and other biological behavior.
