Objective:1.To evaluate the structure and function of right atrium (RA) in patientswith HAF and LAF by AVT.2.To evaluate the ventricular function in patients with LAFby quantitative Tissue Velocity Imaging(QTVI),mitral annular displacement(MAD)andtricuspid annular plane systolic excursion(TAPSE).Methods:1.All persistents with atrial fibrillation were divided into lone atrialfibrillation group(n=36) and hypertension atrial fibrillation group(n=24).Thirty normalvolunteers served as control group.The values of left ventricular were evaluated byM-mode ultrasound.The values of left and right atrium were evaluated by2-DE..RAmaximal area (RAAmax), RA maximal volume(RAVmax), RA minimalvolume(RAVmin),systolic RA filling rate(dv/dtS) and early diastolic RA emptyingrate(dv/dtE) were derived using AVTM.Then the RA emptying fraction(RAEF) wascalculated.2.Thirty-six persistents with Lone atrial fibrillation served as experimentalgroup.Thirty normal volunteers served as control group.The values of right and leftventricular, MAD and TAPSE were evaluated by M-mode ultrasound.Left ventricularejection fraction (LVEF) and right ventricular ejection fraction (RVEF) were calculatedby Simpson.Mitral inflow velocity and tricuspid inflow velocity(E) were measured byDoppler echocardiography.QTVI derived systolic mitral annular velocity,systolictricuspid annular velocity(Vs),diastolic mitral annular velocity,diastolic tricuspidannular velocity(Ve),and the ratio of diastolic mitral inflow velocity to Ve and diastolictricuspid inflow velocity to Ve (E/Ve) were calculated.Result:1.Compared with the values in control group,the RAD1,RAD2, RAAmax,RAVmax,RAVmin and dv/dtE,LAD1,LAD2,LAD3were significantlyhigh and the RAEF,dv/dtS were significantly low in atrial fibrillation group(all P<0.05). There were no differences in LVEDD,LVESD,RVEDD,IVSTd,LVPWTd,LVEF between groups(all P>0.05). The RAD1,RAD2,RAAmax,RAVmax,RAVmin,dv/dtE were significantly higher and the LAD1,LAD2,LAD3,RAEF,dv/dtS were lower in lone atrial fibrillation group compared with hypertension atrialfibrillation group (all P<0.05).2.Compared with the values in control group,the E,E/Ve of left ventricular was significantly high and the Ve,Vs and MAD of leftventricular was significantly low in LAF group(all P<0.05).Compared with the valuesin control group,the E,E/Ve of right ventricular was significantly high and the Ve,Vs and TAPSE of right ventricular were significantly low in LAF group(all P<0.05).There were no differences in IVSTd、LVPWTd、LVEDD、LVESD、LVEF、RVD1、RVD2、RVD3、RVAW、RVEF in all groups(all P>0.05).Conclusion1.The RA structure and function are significantly impaired in patientswith LAF,compared with the patients with HAF.AVT can evaluate the right artialfunction accurately and simply.2.Compared with the control group,the function of LVand RV were impaired in patients with LAF.
