Object:Retrospective analysis of young gastric cancer incidence rate, the proportion of male and female patients, tumor size, tumor location, histological type, blood type, family history, pathology, TNM staging, prognosis, this study will be less than or equal to40years of age in patients with gastric cancer of young patients with gastric cancer is defined according to the2010WHO standard. And at the same time were older than60years of elderly patients with gastric cancer were compared, to explore the clinical characteristics of young patients with gastric cancer, so as to provide basis for early diagnosis and treatment of young patients with gastric cancer.Method:Collected all the implementation of operation treatment, missed the First Affiliated Hospital of Dalian Medical University from December2004to2009December were treated in the Department of general surgery of preoperative adjuvant chemotherapy of primary gastric cancer in1092cases, of which the age less than or equal to40years old of young patients with gastric cancer and72cases of the main object of study, accounted for the proportion of6.5%in patients with gastric cancer. The age is greater than or equal to60elderly patients with gastric carcinoma of323patients in the control group subjects, accounted for the proportion of29.5%gastric cancer patients. For young patients with clinical features and prognosis of elderly patients with survival rate analysis. With other primary malignant tumor, after admission without operation indication to other department of treatment were not included in this group of materials. Using SPSS17.0statistical software for statistical analysis, this paper included percentage, with the median, mean variance comparison, and the related factors were analyzed using the chi square test, p<0.05with statistical significance. Result:1.General information:young gastric carcinoma were72cases, the minimum age of21years, the maximum age39years old, the average age of334.3years, median age34years. Good hair age is30-35years old,28cases, accounting for37.5%.33males, median age33years, mean age344.1;39women, median age34years, average age is324.7, ratio of men to women is:1:1.18.2.The related factors in patients with gastric cancer of young men and women in contrast:young men and women patients with tumor site while in gastric body and antrum of stomach, the incidence of male youth were less than young female patients, the difference has statistical significance; young male patients with gastric cancer staging and typing of the difference between the comparative youth female patients in the endoscopic no statistical significance; TNM the staging of lymph node metastasis, NO male more than female, N3females are larger than males, the difference was significant. Postoperative histopathological classification of male patients with high differentiation adenocarcinoma rare (male to female ratio1:0.22), women with low differentiation adenocarcinoma rare (male to female ratio1:2.36), the difference was significant, showing the tumors of patients with malignant degree of young women than young male patients.3.Comparison of3young patients and elderly patients gender differences:the young group sex ratio (1:1.18) and old age group the proportion of men to women (1:0.56) with statistically significant difference (p<0.05), show the young patients were predominantly female, aged patients were predominantly male.4.Analysis of4young, elderly patients compared to clinical symptoms:young patients with clinical symptoms or abdominal distension discomfort was the most common,68cases (94%); nausea and vomiting, regurgitation in14cases (19%);11patients with dysphagia (15%);24cases (33%); black hemorrhage (27cases of anemia37.5%); inertia weight loss in24cases (33%). The swallowing difficulties symptoms in young patients with gastric cancer is less than in elderly patients. Compared with the p<0.05group, with statistical significance. Other symptoms accounted for the difference ratio of two two compared with no statistical significance. Display the symptom of dysphagia in young patients with rare in elderly patients with gastric cancer.5.Young, elderly patients with tumor position contrast:tumor patients occurred in parts of youth gastric cardia proportion (15.2%) is less than the elderly group (26.9%), statistically significant differences in two groups the proportion of p<0.05. The young patients with gastric tumor in the gastric fundus and cardia of less than older patients.6.Young, elderly patients with tumor pathological features and TNM staging of gastric cancer pathology contrast:youth, low differentiation of undifferentiated adenocarcinoma (51.4%) is greater than the proportion of elderly patients (22.6%), the difference was significant. Young patients with gastric carcinoma, highly differentiated adenocarcinoma (48.9%) less than the proportion of elderly patients, the difference was statistically significant. Young patients with high tumor malignant degree is greater than the proportion of elderly patients. TNM stage:tissues of72cases of young patients with stage T3-T452cases, accounting for the overall proportion of72.2%young patients with gastric cancer. T3-T4in elderly patients with gastric cancer220cases, accounted for68.1%. Comparison of the two groups had no significant difference.7.Comparison of the7tumor size and operation mode:the young group than in young patients with radical resection for50cases, accounting for69.4%; for non radical operation for22cases, accounting for30.1%. Elderly patients undergoing radical resection for201cases, accounting for62.2%, non radical resection for132cases, accounting for40.1%. No statistically significant difference between the two groups of tumor size and operation mode.8.Young patients and elderly patients with genetic history and blood group comparison: the proportion of young patients with genetic history (15.2%) patients older than (6.1%), the difference between the P values of<0.05, with statistical significance, young patients than in elderly patients with family history. The young group and the old group for the same A blood type.9.Young patients and elderly patients5years survival rate:young patients5years survival rate was lower than28.3%of the54.4%elderly patients, there was significant difference between the two groups. Display of youth gastric cancer patients5years survival rate is less than the older patients.Conclusion:1.Young gastric cancer aged30-35years of age, female patients with tumor malignant degree greater than male patients.2.Young gastric cancer tumor location in the gastric antrum, gastric body ministry. TNM staging for stage III, stage IV. The proportion of young patients with lymph node metastasis in N3patients older than. Tumor in young patients in the gastric fundus and cardia less than the percentage of elderly patients. The pathological types of low differentiation adenocarcinoma, undifferentiated higher proportion of elderly patients, in high differentiation, less than the older patients.3.Young patients is higher than the proportion of elderly patients with genetic history.4.Young patients with gastric cancer5years survival rate is less than older patients.
