Objective: To analyze the related factors of premature labor,its delivery ways and theoutcomes of premature infants,in order to provide help for the screening of the high riskpopulation and to develop interventions to reduce morbidity and mortality in preterminfants of high risks.Methods:Chose241cases from the women who were diagnosed with preterm labor inthe Beijing military region general hospital from February2012to April2013(PTLgroup),and260cases of those who have normal deliveries at the same time as thecontrol group, to analyze the related factors of premature labor,its delivery ways and theoutcomes of premature infants.The data including:1. The general situation of mothersand the newborn: age, BMI, the childbirth pregnant weeks, induced abortion history;the Numbers of the newborn, the neonatal birth weight, the gender of the newborn, etc.;2. Monthly income of maternal, active or passive smoking, drinking, etc.;3. Maternalpsychological factors: planned pregnancy and negative event;4. the Antenatalexamination:the first antenatal care, the number of the antenatal care;5. Pregnancycomplications: premature rupture of membranes, gestational hypertension, gestationaldiabetes, multiple pregnancies, abnormal position, placenta previa, placental abruption,inner/surgical complications, uterine factors, abnormal amniotic fluid;6. Delivery ways;7. Neonatal outcome: low birth weight, asphyxia, respiratory distress syndrome (RDS),fetal growth restriction,(FGR) and death. Use independent sample t-test, chi-square test,multi-factor unconditioned Logistic regression analysis, the factors associated withpreterm birth, and the mode of delivery and neonatal outcome of the disease. Results:1. The risk factors associated with gestational hypertension disease:(1) the maternal proportion of the preterm group and control group in monthly income<3000yuan are24.07%and14.23%respectively, the former is higher than the latter,and the maternal proportion of people whose monthly income>5000yuan is18.26%and23.85%, lower than the latter, There are statistical significance between the twogroups (P <0.05); The rate of the preterm group in pregnancy incidence of active orpassive smoking and alcohol consumption is significantly higher than control group (P<0.05); The rate of the preterm group in pregnancy experience negative event is21.99%,in the control group the number is14.62%, the former is significantly higherthan the latter (P <0.05); the proportion of maternal prenatal time>14weeks for thefirst time and the number of <5times in the preterm group is47.30%and34.02%,respectively, the rate in the control group are37.31%and13.85%, the former issignificantly higher than the latter (P <0.05); The probability of The preterm group inthe premature rupture of membranes, gestational hypertension disease, multiplepregnancies, abnormal position, placenta previa, inner/surgical complications, theincidence of uterine factors are43.98%,14.93%,9.54%,7.88%,6.64%,7.47%,12.03%,the proportion of the control group are respectively11.15%,6.15%,1.15%,3.08%,2.69%,3.46%,6.54%, make compare between the two groups, the preterm group issignificantly higher than that of control group (P <0.05);(2) multiple factorsunconditioned logistic regression analysis shows that the number of premature ruptureof membranes, multiple pregnancy,gestational hypertension disease, abnormal positioninto the regression equation; The number of them (OR <1) is the protection factor inpreterm labor, the other four are independent risk factors for preterm birth occurred.2.The influence of preterm delivery way for delivery:The preterm group and the control group maternal cesarean delivery rate are52.70%and38.08%respectively, the former is significantly higher than the latter (P <0.05).3. Preterm birth effects on neonatal outcome: The rate of the preterm group in low birth weight, asphyxia of newborn babies, RDS,FGR, and death are46.89%,19.32%,8.71%,7.19%and4.17%, the control group are4.23%,5.32%,8.71%,1.52%and0.38%, compare the two groups, the former issignificantly higher than the latter (P <0.05).Conclusion:1. Preterm birth is the outcome of combined action of multiple factors, history ofabortion, monthly income, active or passive smoking, drinking, negative event, prenataltime for the first time, placenta previa, inner/surgical complications, uterine factor arethe related risk factors of premature development, premature rupture of membranes,multiple pregnancy, gestational hypertension disease, abnormal position are theindependent risk factors for the preterm birth, the number of antenatal care is theprotective factors for the preterm labor.2. Preterm labor makes the cesarean delivery rate rise.3. Premature infant have a high rate of low birth weight, asphyxia, respiratory distresssyndrome, fetal growth restriction and death.
