Objective: To evaluate the clinical value of the application about binary classificationin epithelial ovarian cancer (EOC) and to analyze the protein expressions of P53、Ki-67、 Dyrk1B、IKKε、Lamininγ1in type I and type II EOC for exploring newmolecular characteristics to distinguish the two types of epithelial ovarian cancer.Methods:146patients with epithelial ovarian cancer were analyzed retrospectively,who were diagnosed and treated in department of gynecology, the First AffiliatedHospital of Dalian Medical University during Jan.2006and April.2013. Thepatients were subdivided into Type I and Type II according to the Dualistic Modeltheory and compared two types of clinical characteristics and the expressions of P53、Ki-67、 Dyrk1B、IKKε、Lamininγ1Results: In146patients, there were61patients with type I EOC, which including11cases of low grade serous carcinoma,5cases of low grade endometrial carcinoma,29cases of mucinous carcinoma, and15cases of clear cell carcinoma.85patients withtype II EOC, which including74cases of the high grade serous carcinoma,11casesof high grade endometrial carcinoma. The mean age of Type I was54.2years old,younger than that of type II (59.7)(p <0.05). Type I in early stage (FIGOⅠ-Ⅱ)accounted for78.7%, which was significant higher than that of type II (P <0.05).Serum CA125level in type I was lower than that in type II (P <0.05).85.2%of Type I EOC involves one side ovary;52.9%of type II EOC involves one side ovaryand47.1%bilateral ovaries (P <0.05). The tumor size of type I was bigger than thetype II (P <0.05). The median survival time of Type I was67.8months, much longerthan the type II (45.0months)(P <0.05). Significant differences in proteinexpressions were found between the type I and type II EOC. In detail, Type I had asignificantly higher expression of Dyrk1B than Type II (vs44.7%, P<0.05),while type II had a significantly higher expressions compared to type I, for P53was11.5%vs58.8%(P<0.05), for Ki-67was19.7%vs43.3%(P<0.05), for Lamininγ1was26.2%vs74.1%(P<0.05but the IKKε expressions were no significantdifference between two tpyes (63.9%vs67.05%, P>0.05). Further analysis of therelationship between immunohistochemical results and survival time, it was foundthat P53, Dyrk1B, IKKε positive cases had a sigificantly poorer survival than thenegative cases (P <0.05), and expressions of Ki-67and Lamininγ1were found tohave little correlation with survival time (P>0.05).Conclusion: Compared with type II EOC, there were several advantage results foundin type I EOC, such as younger age, more early stage, lower levels of CA125, biggertumor size, and longer survival time. Also, there are signigficantly differences inprotein expressions of P53, Ki-67, Dyrk1B and Lamininγ1between type I and type IIEOC, and it can be used as an auxiliary to distinguish the two types of molecularcharacteristics in EOC. Finally, the expressions of P53, Dyrk1B and IKKε byimmunohistochemical detection can be probably served as prognostic indicator forEOC.
