Objective: To investigate the correlation between ischemic cerebral vasculardisease(ICVD) in patients with cerebral artery stenosis and Essen stroke riskscore(ESRS) and related risk factors.Methods:A total of482consecutive patients with symptomatic cerebral arterystenosis who performec cerebral angiography were enrolled in the study.ESRS wasperformed according to the clinical data(Including gender, age, hypertension, diabetesmellitus, previous myocardial infarction, other heart disease, smoking, previous TIA orstroke, peripheral artery disease, low density lipoprotein, triglyceride,cholesterol, highblood fat disease, homocysteine, C-reactive protein, uric acid) of the patients,and thesites of cerebral artery stenosis(anterior circulation,posterior circulation,intracranialarteries and extracranial arteries) were recorded.The vascular stenosis degree wasdivided into mild stenosis10%-49%,50%-69%, moderate stenosis, severe stenosis>70%-100%. If the series is calculated the most serious stenosis stenosis at.The patientswere divided into either a recurrent stroke low-risk group (0-2) or a high-risk group(≥3) according to ESRS.The correlations of ESRS between the degree of cerebralvascular stenosis and sites were analyzed.These risk factors, with vascular stenosis asthe dependent variable,using non conditional Logistic regression analysis of relatedvariables.Results:Of the482patients,198(41.1%)were in the low risk group and284(58.9%) were in the high risk group.⑴The ratios of age,high blood pressure,diabetes,myocardial infarction history,and peripheral arterial disease of thehigh risk group were higher than those of the low risk group.There were significantdifference(P<O.05or P<O.01),while there was no significant difference between thegender and smoking.⑵64(13.3%)patients had mild vascular stenosis,111(23.0%) hadmoderate vascular stenosis,and307(63.7%) had severe vascular stenosis orocclusion.Pearson linear correlation analysis showed that ESRS was positivelycorrelated with the degree of the vascular stenosis(r=0.095,P=0.037). ESRS and theCerebralvascular stenosis were related.The patients with higher ESSEN score showhigher rate of cerebrovascular stenosis.⑶Of the482patients,the patients with anterioror posterior circulation stenosis accounted for52.1%(251/482),anterior circulationstenosis accounted for29.0%(140/482),and posterior circulation stenosis accounted for18.9%(91/482).The patients with anterior and posterior circulation stenosis in the highrisk group accounter for61.6(175/284) and it was higher than38.4%(76/198)in the lourisk group:while the patients with anterior circulantion stenosis was more in the low riskgroup,accounting for40.4%(80/198).There was significant difference(P<O.01).⑷Ofthe482patients,extracranial artery stenosis accounted for45.9%(221/482),intracranial-extracranial artery stenosis accounted for39.4%(190/482),and intracranial artery stenosis accounted for14.7%(71/482).Therewere significant difference(P<O.01).The patients with extracranial artery stenosis andintracranial-extracranial artery stenosis accounted for90.5%(257/284).It was higherthan77.9%(154/198)in the low risk group.(5)With age, hypertension, diabetesmellitus, previous myocardial infarction,other heart disease, smoking, previous TIA orZu Zhong, peripheral arterial disease, low density lipoprotein, triglyceride, cholesterol,highblood fat disease, homocysteine, C-reactive protein, uric acid and otherrisk factorsas variables, in order to vascular stenosis as dependent variable, using non conditionalLogistic regression analysis, gradually eliminating the non significant variables,regression model finally left thevariables of hypertension, heart disease, other Gao Zhiin history, uric acid.High blood presure,other heart disease(except for myocardialinfarction and atrial fibrillation),the history of hyperlipoidemia and hyperuricemia were closely related with cerebral vascular stenosis.Conclusions:The degree of cerebral artery stenosis in ischemic cerebrovasculardisease was associated with ESRS, the distribution of the sites of cerebral artery lesionswas different when ESRS was different.High blood presure,other heart disease(exceptfor myocardial infarction and atrial fibrillation),the history of hyperlipoidemia andhyperuricemia were closely related with cerebral vascular stenosis.
