[Objective] Systemically review the reduction of juvenile orthodontic complication by self-ligating bracket in nearly a decade. Draw a conclusion by Meta analysis in the controversial related research filed. Find deficiency in the previous study and provide scientific basis for clinical treatment[Methods]Computer retrievals were reviewed on PubMed (2003-2013.5), Embase (2003-2013.5), the Cochrane Library (2003-2013), CBM (2003-2013.5), CNKI (2003-2013.5), VIP database (2003-2013.5) and so on. The inclusion criteria was whether the self-ligating bracket can reduce the orthodontic complication when compared with the randomized controlled group, including orthodontic pain, periodontal damage, and root absorption. Rev Man5Meta analysis software was used to find which one would be better.[Results]1. The relationship between self-ligating bracket and orthodontic pain:the systematic review included eight studies and the total cases were381. Meta-analysis results indicate that useing self-ligating bracket bracket could reduce the pain when compared with the traditional bracket. The VAS score difference was statistically significant[MD=-0.35,95%CI (-0.92,-0.4) P<0.001].2. Self-ligating bracket and periodontal damage:The systematic review included six studies and the total cases were168. Meta-analysis results indicated that using self-ligating bracket bracket reduced the periodontal damage. The difference was statistically significant. GI [MD=-0.43,95%CI (-0.55,-0.32) P<0.00001] SBI [MD=-0.43,95%CI (-0.59,-0.27) P<0.0001], PLI [MD=-0.72,95%CI (-0.83,-0.60) P <0.0001]3. Self-ligating bracket and root resorption:The systematic review included four studies and the total cases were218. Meta-analysis of the results indicated that using self-ligating bracket had no difference when compared with the traditional bracket after treatment. The difference was not statistically significant.[MD=-0.01,95%CI (-0.21,0.19) P=0.93][Conclusion]Compared with the traditional straight wire bracket, self-ligating bracket would get less pain, less periodontal damage. However, it did not show a clear advantage in the lower root absorption degree. Clinicians in selecting braces can refer to this study. Whether the results are relevant to the correct using of the arch wire, the malocclusion styles and the treat plan is also needs further clinic researches.
