ObjectiveChildhood abuse is a severe international issue., especially depression is a common and typical sequelae. Anxiety associated depression, and/or depression with anxiety is charactered with severe symptoms, chronic process, serious social dysfunction, high suicide rates and poor prognosis, therefore, some researchers suggest that anxiety associated depression is a third mental disorder, and is different to pure anxiety or depression. A control study is carried out to examine the clinical and psychological characters among anxiety associated depression with and without childhood abuse patients. This study aims to explore these patients’clinical features and personality characters, therefore offer theoretical evidence of clinical intervention, individual therapy, disease prevention and further research.MethodsA case-control method was taken among86anxiety associated depression patients,36cases with childhood abuse and controlled with50cases without childhood abuse; all of them match the depression criteria of International Classification of Diseases10edition (ICD-10). i Entry criteria:HAMD (24) total score≥20points; HAMA(14) total score≥14points.The formal consent were obtained from participants or their families. The general information questionnaire, HAMD, HAMA, CECA-Q, MMPI, and LES were all used to evaluate the participants, P<0.05was considered as statistically significant.Results1.Clinical phenomenological aspects:childhood abuse is more common in female patients, at early age onset; family histories are similar; clinical symptoms were more severe compared with patients without childhood abuse; a high incidence of sexual symptoms were appeared in the biological factors; there is a high rates of cardiac and vegetative nerve dysfunction; There is a high incidence of suicidal ideation or behavior; scores of anxiety/somatization, cognitive dysfunction, hopelessness are significantly higher in the anxiety associated depression patients who have childhood abuse.2.Social psychological aspects:personality assessment shows that childhood abuse patients indicates the unstable emotional personality, easy to impulse, attack, hostile attitude and suicide. The personality of patients with childhood abuse was charactered as high psychoticism, high neuroticism, non-socialized, non-obvious extroversion, exaggeration, low EQ and ability of decision-making, tend to be violent, traffic offenders, substance abuse users, and anti-social tendencies. In the way of defense, the patients with childhood abuse are likely to use immature psychological defense mechanisms:denial, flinch, transfer and projection, often appears naive, impulsive, shirk responsibility. There is no significant difference in the score of negative life events between patients with childhood abuse and without childhood abuse. However, the differences exist in the family-related issues, and the patients with childhood abuse are higher than the patients without childhood abuse.3.Emotional neglect and sexual abuse are the risk factors for the onset of anxiety associated depression among patients with childhood abuse, therefore, childhood abuse is related to anxiety associated depression; emotional neglect and sexual abuse play an important role in developing the disease.Conclusion1.Patients of anxiety associated depression who is with childhood abuse is more likely to onset in female, at an early age, with severe clinical symptoms. There is a high incidence of cardiac symptoms, sexual symptoms and autonomic dysfunctional symptoms, cognitive disorders, hopelessness, suicidal ideation or behavior.2.The personality is likely to be emotional unstable, prone to hostility, impulsive, offensive and suicide among patients of anxiety associated depression with childhood abuse. Their EQ and decision-making ability is relatively low, anti-social personality. Most patients with childhood abuse tend to take immature psychological defense mechanisms, and have more negative family-related issues.3.Childhood abuse is related to the anxiety associated depression, emotional neglect and sexual abuse play an important role in developing the disease.4.The experiences of childhood abuse may lead to more serious clinical consequences, it is very important to deal with the negative experience in the psychological therapy, make individual plan, improve their immature defense style, their hopelessness feeling, help to reduce the risks of suicide, promoting the rehabilitation, and prevention.
