Background:Klotho (kelaosu) gene from Kuro and friends found in the study.Its name is based on available human lifeline Greece goddess.After further research to find out the Klotho gene product of the Klotho protein, the main distribution and expression in brain and kidney of choroid, divided between secreted and membrane-type2types of protein. Play a dominant role of secretory proteins in human.Klotho was confirmed by experimental study on gene associated with aging. Was successful in cloning success in mice. The gene expression of rat (Klotho-/-) phenotype is similar to human aging [1-3], that is, mice (klotho-/-) demonstrate reduced life expectancy, reductions in pulmonary emphysema, infertility, osteoporosis, atherosclerosis, pituitary abnormalities, skin atrophy, gait disorders, abnormal ectopic calcification, osteoporosis and calcium metabolism [4]. Description the hormone Klotho gene is associated with aging, and aging is closely related to the occurrence of diseases of aging and development plays an important role.Klotho gene and disease relations in recent years attention has come under domestic and international scholars, now there is a lot of literature confirms Klotho relationship to aging. Now with the coming of aging society, more and more unavoidable for surgery and anesthesia in elderly patients, while older patients due to recession of its own organs, resulting in all kinds of complications after surgery or anesthesia, especially in the POCD is most common. But as anesthesia for a worker is not aware of their relationship with anesthesia.Purpose:This project aims at cellular and molecular level study on mechanism of occurrence of POCD in elderly patients to explore the effects of different anesthetic methods on the Klotho Klotho gene expression protein has relationships to reduce the incidence of POCD whether elderly patients for our study of occurrence of POCD in elderly patients for the new molecular biological mechanisms.Methods:Choose general anesthesia ASAI-II level to be above the age of65of70cases senile patients, male,38, female to32cases. Former no history of surgery and diseases of the central nervous system. Such balloting method in patients were randomly divided into2groups, group-intravenous anesthesia with full set of labeled T, inhalation anesthesia group labeled group I. Induction of anaesthesia:induction of anesthesia:0.1-0.15mg/kg Midazolam, fentanyl0.002-0.004mg/kg, etomidate0.15-0.3mg/kg, Vecuronium Bromide ammonium O.lmg/kg. Intraoperative anesthetic maintenance: Group T via micro-pump for propofol3-6mg·kg-1·h-1, remifentanil0.05-0.20μg/g·kg-1·min-1, clinical Vecuronium continuous drug delivery;, group I based on patient weight used for indicators, such as three to isoflurane inhalation and Vecuronium and also in continuous delivery. Group T depending on the patient’s blood pressure after the surgery began, heart rate adjust propofol/remifentanil dose, increase or decrease the dose; and groups equally depending on the patient s blood pressure, heart rate and adjust the dose of inhaled anesthetics isoflurane, select increase or decrease the volume. And intermittent injection of Vecuronium Bromide in maintaining muscle relaxation to meet surgeon surgery needs. Respectively set as T1before anesthesia, surgical anesthesia after the withdrawal is set to the end of T2two time points from venous blood, determination of klotho protein content by using ELISA method. In preoperative and postoperative patients after1day,2hours,1th day,3rd day after surgery on senile patients with simple mental status examination form (Milli-Mental State Examination,MMSE) for tests of cognitive function.Result:1.The incidence of postoperative1day POCD is37.88%,the third day almost back to normal2.After ELISA technique for measuring of Klotho protein content down from preoperation, and POCD occurred in patients after surgery decreased Klotho protein content.Conclusions:The occurrence of postoperative cognitive dysfunction may be related to Klotho gene. Intravenous and inhaled anesthesia methods can be led to a decline in patients Klotho proteins in the body, caused by inhalation anesthesia in elderly patients with Klotho protein values fell more than intravenous anesthesia.
