ObjectiveThis thesis aims to explore the standardization and objectivity of ankylosing spondylitis TCM syndrome types through statistical methods. It analyzes clinical symptoms of ankylosing spondylitis by using cluster analysis, tries to discover the relationship and regularity between symptoms and makes rational typing. Based on it, this paper explores the correlation factors associated with TCM syndrome type of ankylosing spondylitis, attempts to establish and improve standardized TCM differential treatment system of ankylosing spondylitis and provide an objective basis for it. This lays a foundation for further multi-center study and strives to establish a syndrome type standard with universal significance as early as possible.MethodsIn strict accordance with research diagnostic criteria for ankylosing spondylitis and inclusion and exclusion criteria to select patients,108qualified patients are selected in the outpatient and inpatient department from Third Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine, Guangzhou Orthopedic Hospital, and the first Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine. Then collect patients’syndrome information, record data and use cluster analysis method to group and analyze ankylosing spondylitis. Divide the patients into several types in combination with clinical practice. Based on it, further explore the relationship of correlation factors with syndromes, including age, gender, other general information, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), immunoglobulin (Ig), platelet (PLT) and other laboratory parameters. ResultsCluster analysis outcome of ankylosing spondylitis syndromes:The study finalizes four types which is most reasonable and consistent with clinical reality. They are moist cold arthralgia syndrome (type1), moist heat arthralgia syndrome (type2), blockage of phlegm and blood stasis syndrome (type3) and deficiency of liver and kidney syndrome (type4), in which the heaviest proportion is type2accounting for38.89%, followed by type3accounting for27.78%, type4accounting for19.44%and type1accounting for13.89%.Study outcome of ankylosing spondylitis syndromes with correlation factor: The difference between age and four syndromes shows no statistical significance (P>0.05). The difference between ESR and AS syndrome groups shows statistical significance by single factor of ANOVA (P<0.05). On this basis, the difference between type1&2&3and type4shows statistical significance by further comparison of each two different syndromes (P<0.05). The difference between type1, type2and type3shows no statistical significance by each two different syndromes comparison (P>0.05). Through nonparametric rank sum test, the difference between C-reactive protein and AS four syndromes in non-parametric case shows statistical significance (P <0.05). After further comparison of each two groups on this basis, the difference between type2&3and type4shows statistical significance (P <0.05). The difference between other groups shows no statistical significance by each two syndromes comparison (P>0.05). The difference between platelets and AS TCM shows statistical significance through single factor ANOVA (P <0.05).On this basis, the difference between type3and type2&1&4shows statistical significance by further comparison of each two groups (P <0.05). Other comparison shows no significant difference (P>0.05). The difference between IgA, IgG, IgM and AS four syndromes shows no statistical significance (P>0.05). Conclusion(1) This research establishes AS TCM syndromes which are moist cold arthralgia syndrome, moist heat arthralgia syndrome, blockage of phlegm and blood stasis syndrome, and deficiency of liver and kidney syndrome, in which the heaviest proportion is moist heat arthralgia syndrome. The main reason is that most patients selected are Lingnan people who live in Lingnan with more humid, less dampness climate. The heat-humid physical factors, research with small sample and single region studies are influencing factors.(2) Male to female ration is4:1among enrolled patients with age range from16to55years old and average age of26.84±6.79years old. It shows no significant correlation between age and AS four syndromes.(3) ESR, CRP levels of moist heat arthralgia syndrome increases most significantly and PLT level of blockage of phlegm and blood stasis syndrome increases most obviously. There is no significant correlation between IgA, IgG, IgM and AS four syndromes.
