The Effect Of LiuweiDihuang Decoction On Controlling Variability Of Blood Pressure In Elderly Hypertension

ObjectiveThis study objectively evaluate the clinical effect of LiuweiDihuang Decoctionin in elderly patients with hypertension characterized as deficiency of Yin and hyperactivity of Yang. In addition, the study explore its effects of the medicine on the blood pressure variability.Methods60cases Of elderly patients diagnosed as hypertension were collected. All of the cases met Traditional Chinese medicine(TCM) diagnosis criteria and Western medicine diagnostic criteria. Then the cases were divided randomly into treatment group and control group, each with30cases. The two groups must have no significant differences in age, sex, general pathogenic condition etc. Patients in treatment group were given treatment of amlodipine combined with LiuweiDihuang Decoction. Patients in control group were given treatment of amlodipine exclusively. The two groups were monitored for4weeks to observe the change in the level of blood pressure, ambulatory blood pressure related indicators, TCM symptoms integral before and after treatment. The data were analyzed using statistical method to evaluate therapeutic effect.ResultsAfter treatment, therapeutic effect characterized as lowering blood pressure was observed in patients from both the treatment and the control groups (P<0.05), comparison of both systolic blood pressure (SBP) and diastolic blood pressure (DBP) showed no difference between groups (P>0.05), significant difference was observed for pulse pressure (PP) between groups (P<0.05). On24hour systolic blood pressure standard deviation (24SBP-SD), Daytime systolic blood pressure standard deviation (DSBP-SD) and Nighttime systolic blood pressure standard deviation (NSBP-SD), the significantly decrease of two groups was observed (P<0.05); The results showed that the two groups could reduce systolic blood pressure fluctuation and the treatment group was superior to control group. but the24hour diastolic blood pressure standard deviation (24DBP-SD), Daytime diastolic blood pressure standard deviation (DSBP-SD), Nighttime diastolic blood pressure standard deviation (NDBP-SD) also decreased in treatment group (P<0.05), while the control group had no difference (P>0.05). Except on Nighttime systolic blood pressure variability coefficient (NDBP-CV),24hour systolic blood pressure variability coefficient (24SBP-CV),24hour diastolic blood pressure variability coefficient (24DBP-CV), Daytime systolic blood pressure variability coefficient (DSBP-CV), Daytime diastolic blood pressure variability coefficient (DDBP-CV), Nighttime diastolic blood pressure variability coefficient (NSBP-CV) were observed significantly decrease for of treatment group(P<0.05);but these differences were not observed in the control group. Compare with two groups, the SBP-CV, DBP-CV of treatment group decreased more significantly than control group at any time in a day(P<0.05).By analyzing TCM total symptom score and TCM syndrome curative effect of groups, the total efficiency showed96.67%in treatment group, while control group was73.33%, and treatment group was superior to control group. Comparison on the single symptom with two groups, the symptoms of soreness of waist, soft knee, agrypnia, dysphoria in chestpalms-sole, palpitation were improved more significantly in treatment group than control group (P<0.05), but dizziness, headache, tinnitus, forgetfulness showed no difference in groups. Throughout the clinical test, there were no adverse event happened in two groups which concerned with experimental drugs, the safety index showed no difference after the test.ConclusionIn the observation, the LiuweiDihuang Decoction is capable of reducing the pulse pressure, controlling blood pressure variability, decreasing blood pressure fluctuations, and significantly improving the clinical symptoms. The therapeutic risk of the medicine is low and deserved further investigation and clinical application.
