| Objective To evaluate the relation between retinal vessel abnormalities andischemic white matter lesion.Methods We searched Cochrane Library (Issue4,2013), Medline (1950to2013), Embase (1980to2013), CBMdisc (1978to2013), CNKI (1979to2013), VIP(1989to2013), Wan fang Database (1978to2013), CMCA (1994to2013) byelectronic database to collect case-control studies about the relationship betweenretinal vessel abnormalities and ischemic white matter lesion. The quality of includedstudies was assessed according to the criteria recommended by the CochraneHandbook for Systematic Reviews of Interventions and data were extracted by tworeviewers independently. We made meta-analysis, quantitative qualitativecomprehensive analysis, funnel plot analysis for the study results. In themeta-analysis, chosen fixed or random potency model according to the heterogeneity.Using the Cochrane Collaboration’s RevMan5.0software conducted Meta analysis.95%confidence interval etimated the evaluation index and made α=0.05for the sizeof test.Results We research bring in4literatures and got a sample size of2159,including495cases in case group and1664cases in control group. The incidence ofretinal vessel abnormalities was3.04-fold [OR=3.26,95%C(I2.28,4.06),P<0.00001]higher in WMLs group than in non-WMLs group. Comprehensive literatures thatincluded in this study, select from the following indicators of retinal vesselabnormalities: arteriovenous crossing, exudates, arteriolar narrowing, retinalhemorrhage and micoaneurys. The risk of suffering from WMLs is2.26-fold higher[OR=2.26,95%CI (1.70,3.00),P<0.00001]with people that presence ofarteriovenous crossing than people without this factor(arteriovenous crossing), Therisk of suffering from WMLs is2.52-fold higher [OR=2.52,95%Cl (1.56,4.07),P=0.0002] with people that presence of exudates than people without this factor(exudates), The risk of suffering from WMLs is2.55-fold higher [OR=2.55,95%CI(1.51,4.30),P=0.0004] with people that presence of retinal hemorrhage thanpeople without this factor(retinal hemorrhage), micoaneurys [OR=1.56,95%CI(0.36,6.72),P=0.55] impact on WMLs incidence was no significant.Conclusion Exposed retinal vessel abnormalities are more likely to occurWMLs, compared with those not exposed. Arteriovenous crossing, arteriolarnarrowing, exudate and retinal hemorrhage were associated with the occurrence ofWMLs, micoaneurys impact on WMLs incidence was no significant. |
