Objective: To analyze the characteristics and risk factors for malignant tumors inpatients with type2diabetes mellitus (T2DM).Methods: This study consisted of741cases of T2DM patients with malignant tumorand70cases of T2DM patients without malignant tumor in the Second AffiliatedHospital of Dalian Medical University from January2011to March2014. The patientswere divided into three groups as Group A, B and C. In Group A, there were141casesfrom Endocrinology department who were diagnosed T2DM with complete clinical data.In other departments,600cases of T2DM patients complicated with malignant tumors(regardless of the order of T2DM with malignant tumor) were classified in Group B. Ascompared with Group A, the patients’ sex and tumor site were collected and beinganalyzed for the distribution characteristics of malignant tumors. In confirming the riskfactors of malignant tumors in T2DM patients, random samples of70cases of T2DMpatients without malignant tumor from Endocrinology Department were taken andregarded as Group C, with the same complete clinical data as Group A.Results:1. There were several cases of T2DM complicated with malignant tumor inwhich, majority were colorectal cancer, followed by lung cancer, liver cancer, stomachcancer and thyroid cancer. In group A, top most was the thyroid cancer, alongside withlung cancer, colorectal cancer and breast cancer. In Group B where in the T2DMpatients complicated with malignant tumors, mostly were colorectal cancer followed bylung cancer, stomach cancer and liver cancer;The incidence rate of thyroid cancer forfemale was higher than male. With regards to cancers in colon, lung, liver and stomach,the occurrence rate for male were higher than those for female, except for breastcancer (in this study were female) and male and female reproductive system cancer whichare gender-specific. The difference was statistically significant (P<0.001);Theprevalence of thyroid cancer in Endocrinology was significantly higher than the otherdepartments in the hospital (P<0.001).2. T2DM patients who were present with malignant tumors were concentrated in theage group of51-80years old. There was no significant difference in age among maleand female patients (P>0.05), and the highest incidence is in the five years courses ofdiabetes.3. T2DM patients those without drug treatment, the sick time of malignant tumorsearlier than oral medications group (P<0.001) and insulin therapy group(P=0.007). Thesick time of malignant tumors of insulin therapy group earlier than oral medicationgroup (P=0.010).4. Analysis on the single factors for development of malignant tumors in T2DMpatients for a higher risk of malignant tumor (P<0.05) are: age≥50years old, smoking,BMI≥25,5years duration of T2DM, and insulin treatment patients.5. Unconditional logistic regression analysis for development of malignant tumors inT2DM patients for independent risk factors of malignant tumor are: age, smoking,5years duration of T2DM, and insulin treatment.Conclusion:1. Patients with T2DM are often complicated with different malignanttumors, such as colorectal cancer, lung cancer, liver cancer, stomach cancer, thyroidcancer, and breast cancer; male patients were mostly found with colorectal cancer, lungcancer, liver cancer, and stomach cancer and female patients were with thyroid cancerand breast cancer; clinicians should be concerned more about the incidence of thyroidcancer in female with T2DM.2. The risk factors for development of malignant tumors in T2DM patients are so manybut followings are independent relevant: age, smoking,5years duration of T2DM, andinsulin treatment.
