Study The Treatment Strategies Of Propranolol For Infantile Hemangioma

Objective:For young children under3,hemangioma is a kind of common benign tumors characterized by vascular endothelial cell proliferation. It occurs primarily in the body surface like head, limbs and so on.Also,it often happens in mucous membrane, muscle, bone, and even intracranial and other special parts. The natural process of IH includes hyperplasia and fade. Although there is a natural fading characteristics in IH, it should be timely treated for life-threatening or may cause dysfunction hemangioma. At present,there is a variety of treatments for IH including steroids, chemotherapy drugs, laser, hardener, surgical removal, etc. Now there is still no evidence to show that exactly which approach is better for the treatment of IH. The steroid hormone was used as the first-line treatment drugs Before propranolol reported in2008. Since propranolol in the sights of clinicians, whether it is worth wide application is one of the focuses of debates in the long term. This article aims to study the clinical application of propranolol in the treatment of IH cases.We focus on the application dosage and adverse reactions to explore the safety of oral propranolol in IH for providing strong support in this area.Method:We collected a total of47patients with hemangioma seeing a doctor in our hospital or other3armour hospital in kunming from january2013to december2013as the research object. Depending on the dosage of propranolol,17cases were divided into test group using the oral dose of1.0mg/kg. d,2times a day.20cases were divided into the control group using the oral dose of2.0mg/kg. d,2times a day.10 cases were divided into the placebo group.The treatment time for all the three groups was6months.Also, we continued to follow-up these cases1month after the drug was stopped.We used the naked eye observation measurement and color doppler ultrasound as the curative effect evaluation criteria and whether relapse.During this period,we also monitorred the patients’ blood pressure, heart rate, general mental state etc closely.Results:To some extent,the tumor became smaller after taking this medicine both the test group and control group cases. And the heart rate turn to slow down between83and110times per min. The heart rate of3cases slow down obviously in control group as low as83-89times per min during sleep.While,the3cases could restore automatically after awakening.So,these cases were not special treated. Taking the medicine in the first week,6cases of test group happened mild diarrhea,and3cases happened anorexia.These cases were alleviated by themselves.8cases of control group happened diarrhea,6cases happened anorexia, and2cases happened changes of sleep as giving priority to with sleep time increased. These cases that had adverse effect could finish the study after corresponding treatment.37cases had healed after1month. No obvious hypoglycemic reaction was found.Conclusion:1、It is effective to use oral propranolol in infantile hemangioma; Drug adverse reaction was larger in the test group during early time; After1month of medication, the color becomes shallow in two groups of children with a total of37cases,and the surface performance of tumors shrink, but curative effect comparison shows that the control group patients was obvious;2、After3months,the drug adverse reactions were not observed in3group cases, and there is no difference of two groups in curative effect;3、Oral higher dose of propranolol for long term is more effective towards IH in the region.
