ObjectiveThe study is designed to investigate the clinical effectiveness and feasibility of ear-apex bloodletting coupled with traditional electro-acupuncture therapy for excessive syndrome of migraine, through observing its immediate, short-term and long-term effects.MethodsBased on the principle in randomized controlled trials,60patients of excessive syndrome of migraine were randomly assigned to two groups of30volunteers:â‘ observation group, treated with ear-apex bloodletting and electro-acupuncture;â‘¡control group, treated with traditional electro-acupuncture. Both groups were treated by needling on affected side of taiyang, shuaigu, xuanlu, touwei, fengchi, waiguan, taichong, zulinqu, and then adjusted the points according to different symptoms. Control group used traditional electro-acupuncture treatment which last for30minutes, once a day for6days. Observation group used ear-apex bloodletting on one tip after finishing traditional electro-acupuncture, once a day, alternating sides. In the first treatment, the VAS ratings were observed at the baseline and after treatment of0hour, half an hour,1hour. Headache index, Migraine degree of disability assessment scores (MIDAS), World Health Organization Quality of Life Scale Brief Version (WHOQOL-BREF) were assessed at baseline, after6times of treatment and three months after the whole treatment. The data were input to SPSS19.0for analysis, and evaluate the immediate, short-term, and long-term effect by using the ear-apex bloodletting to treat excessive syndrome of migraine.Result 1. Baseline information:the analysis by gender, age distribution, illness duration severity and handicapped degree show that there is no statistically significant difference in two groups which meant that two groups were comparable.2. Immediate analgesic effect:the analysis of VAS score in baseline, after treatment of0hour,0.5hour,1hour shows statistically significant difference in0.5hour and1hour after treatment (P<0.5) while shows no significant difference in0hour after treatment (P>0.05). That means the two groups have similar analgesic effect in0hour after treatment. And in0.5hour and1hour after treatment, observation group have better analgesic effect than control group. The analysis of remission rate of headache in1hour shows statistically significant difference in two groups, and observation group is better than control group.3. Short-term effect:compared with baseline, both groups have improvement in attack times, pain extent, lasting time, accompanying symptoms, headache index and curative effect. Result shows that observation group have better improvement in attack times, pain extent, lasting time, headache index and curative effect while improvement of accompanying symptoms is the similar in two groups.4. Long-term effect:The analysis of3months follow-up shows that both groups have an improvement in terms of the Headache index, curative effect, MIDAS scores, WHOQOL-BREF scores compared with baseline, and comparison between two groups has significant differences. The result also shows that the improvement of long-term headache index, curative effect, the degree of disability and the quality of life of the conservation group are more obvious than control group.ConclusionBoth ear-apex bloodletting coupled with electro-acupuncture therapy and traditional electro-acupuncture therapy have a good therapeutic effect on improving immediate analgesic, the headaches index, curative effect, the degree of disability and the quality of life. But comparing two therapies, the ear-apex bloodletting coupled with electro-acupuncture therapy is superior to traditional electro-acupuncture therapy. In conclusion, the ear-apex bloodletting coupled with electro-acupuncture therapy is worth of being generalized in the treatment of excessive syndrome of migraine.
