| ObjectiveTo investigate the expression of sphingosine kinase1(SPHK1) and nuclear factor kB (NF-kB) in non-small cell lung cancer (NSCLC)and their relationships with invasion, metastasis and prognosis of non-small cell lung cancer.Methods93non-small cell lung cancer specimens and normal lung tissue from conventional surgery were confirmed by pathological diagnosis in Tianjin Chest Hospital from January to December in2009. all cases had not been treated with radiotherapy and chemotherapy before their surgeries. Expression of SPHK1and NF-kB p65in93cases paraffin sections of non-small cell lung cancer were detected by Immunohistochemistry method. According to the patients’survival status from follow-ups, we could study the relationship between expression of SPHK1and prognosis.32cases had been collected from fresh specimens of non-small cell lung cancer in Tianjin Chest Hospital in2013, We could use RT-PCR to detect the expression of SPHK1mRNA in non-small cell lung cancerResult1. In non-small cell lung cancer the positive rates of SPHK1, NF-kB P65expression (respectively96.8%and89.2%)were higher than the positive rates (18.3%and12.9%) of those in adjacent normal lung tissue, and there was statistical significance (χ2=117.259and(χ2=108.459, respectively, both P<0.01).The rates of high expression of SPHK1, NF-kB P65were50.3%and47.3%respectively, The high expression of SPHK1and NF-kB was positively correlated in non-small cell lung cancer (r=0.464, P<0.01).2. The differences of SPHK1and NF-kB p65expression were not associated with the patient’s age, sex, tumor size, tumor location, histological type and degree of differentiation (all P>0.05). The higher TNM stage was, the proportion of SPHK1and NF-kB p65expression was higher, and this difference was statistically significant (χ2=19.738,χ2=25.552, both P <0.01);The ratio of SPHK1and NF-kB p65high expression in the group of lymph node metastasis was higher than the proportion of those without lymph node metastasis, and the difference was statistically significant (χ2=12.573, χ2=11.742, both P<0.01); SPHK1and NF-kB p65expression ratio in the group of death was higher than those in the group of survival, and the difference was statistically significant (χ2=9.389,χ2=23.626, both P <0.01).3. According to the patients survival status from follow-ups, we could get the result that overall median survival time was33months in total44months. The mean survival time was39months and median survival time was38months in the group of SPHK1low expression; The mean survival time was27months, median survival time was21months in the group of SPHK1high expression. The survival rate was73.91%in the group of SPHK1low expression and was42.55%in the group of SPHK1high expression, the difference was statistically significant (Log-rankχ=14.025P<0.01)4. RT-PCR results showed that expression of SPHK1mRNA (0.55±0.18) in lymph node metastasis samples than expression of SPHK1mRNA expression (0.27±0.02) in those samples without lymph node metastasis, and this difference was statistically significance (t=-6.487P<0.01).The expression of SPHK1mRNA (0.66±0.12) in the group of stage â…¢ was higher than those (0.29±0.05)in the group of stage â… and â…¡, and this difference was statistically significant (t=-12.110P<0.01)Conclusion1. The expression of SPHK1, NF-kB p65in non-small cell lung cancer was higher than those in normal lung tissue, and the expression of SPHK1and the expression of NF-kB P65in non-small cell lung carcinoma were positively correlated.2. The differences of SPHK1and NF-kB p65expression were not associated with the patient’s age, sex, tumor size, tumor location, histological type and degree of differentiation. The differences of SPHK1and NF-kB p65 expression were associated with TNM stage, lymph node metastasis and survival status of patients. The higher TNM stage was, the expression of SPHK1and NF-kB p65was higher, that expression of SPHK1and NF-kB p65in lymph node metastasis samples than expression of SPHK1and NF-kB p65in those samples without lymph node metastasis.The expression of SPHK1and NF-kB p65in the group of death was higher than those in the group of survival. The expression of SPHK1in gene level was associated with TNM stage, lymph node metastasis.The above research indicated that SPHK1could be used as an evaluation index of non-small cell lung cancer invasion and metastasis and might exert its effects through the NF-kB.3.
