ObjectivesBy prospective randomized controlled trial design, applying traditional Chinese medicine five phases of music to relieve premature pain, observe the heel blood pain stimulation premature physiological indexes, the change of the respect such as pain score. At the same time, the comparison of traditional Chinese medicine five phases (Jue yin) and western music (Chopin’s serenade) in alleviating heel blood pain differences, infant pain intervention countermeasures, for neonatal screening blood disease to provide personalized treatment.MethodsThis subject adopts the design method of prospective randomized controlled trial, in March2013to January2014in the Nan hai maternity children hospital affiliated to Guangzhou university of Chinese medicine neonatal intensive care unit (NICU) included in the standard or greater gestational age32weeks and37weeks, a total of90cases of premature infants were randomly divided into3groups:traditional Chinese medicine five phases of30cases of music, western music (Chopin’s serenade)30cases, blank control group30cases. In the heel blood of western music(Chopin’s serenade), traditional Chinese medicine five phases (Jue yin), by neonatal pain assessment scale (NIPS) and premature pain score summary table (PIPP) evaluation neonatal heel pain in pursuance of the behavior score, crying duration, the incidence, the incidence of minimal or no pain don’t cry. Combining scale observation of three groups of music before and after treatment in children with clinical symptoms and signs, and monitoring records before and after treatment respectively-breathing, heart rate, crying duration, SP02and NIPS, PIPP pain score evaluation index of neonatal pain physiological and behavioral. At the same time, for the treatment of neonatal play music (born after72hours) before and3days after music therapy for rapid auditory brainstem evoked potential (AABR) check, for safety assessment.Results1, Combining scale observation of three groups of children music before and after the intervention of clinical symptoms and signs, and play music in heel blood stimuli and does not play music in pain rating scale (NIPS, PIPP pain score), breathing, heart rate, crying duration, the change of the SP02difference had statistical significance (P<0.05);2, The traditional Chinese medicine five phases of music (Jue yin) from western music (Chopin’s serenade) in reducing pain stimulation caused by oxygen desaturation, reduce pain face duration, reduce the PIPP score was statistically significant (P<0.05), and in reducing the heart rate, respiration and NIPS scores for both groups has no statistically significant difference (P>0.05).3, Before and after treatment for newborns rapid auditory brainstem evoked potential (AABR) check, all without exception. Blood does not appear in the process of syncope and bleeding, no subcutaneous petechiae, and ecchymoses adverse reactions.Conelusion1, Play music in the process of premature heel blood can effectively alleviate the pain.2, Jue yin to reduce premature heel blood pain stimulation caused by oxygen desaturation, reduce pain face duration, reducing PIPP scoring more obvious effects than western music.3, Music therapy is safe and practical.
