Objective:Assess the activity of cathepsin D(CAT-D)and alpha-1antitrypsin (AAT) in blood in patients with osteoarthritis with hip or knee osteoarthritis. To explore Whether these two enzymes could turn into serum biochemical indicators of detecting cartilage degeneration.Methods:The activity of CAT-D and AAT in blood serum of44women and26men with hip or knee osteoarthritis was determined with the method of ELISA before total joint replacement, on the tenth day after surgery, and once in100healthy patients.Hip osteoarthritis in54cases and knee osteoarthritis in16cases, both involving a single joint.. By age group,27cases between40to59,60-74between37cases,6cases between75-89. Of which36people with type2diabetes,40patients, with hypertension.Healthy group100cases, of which36men, women,64,36cases between40to59,60-74between55and75-89between9people.All the data using SPSS19.0software for research.Results:The preoperative activity of CAT-D in patients with osteoarthritis was lower by25%(ng/ml,P<0.05) and after surgery by50%(ng/ml, P<0.05).Simultaneously, the preoperative activity of AAT in the OA (osteoarthritis) patients postoperative was by80%higher (ng/ml P<0.001) than in healthy patients.And the Gender, hypertension, diabetes have no effect on two kinds of enzyme activity. above (P>0.05), Also not associated with age.Hip and knee osteoarthritis have no difference between two enzymes activity.(P>0.05).Conclusion:The low CAT-D activity in osteoarthritis of big joints is associated with a decrease of cartilage cells during the degenerative process. The higher activity of acute phase protein AAT in OA postoperative patients’ blood serum confirms the possible inflammatory component in the osteoarthritis.And the two enzymes are not affected by gender, age, hypertension and diabetes, etc.This study also found no differences between these two kinds of enzyme activity in Hip and knee osteoarthritis.So We hope the two enzymes could become serum biochemical indicators to detect cartilage degradation process.
